The weirdest monster I've seen in a while was the Shism from Silent Hill:Homecoming. Believe me, I thought it looked like a banana before I even saw the ZP review. That or the Centaurs from Fallout 3.

I remember playing this game when I was like, 8 or something, and I pretty much freaked out on the final level. You know, the one where you've gotta run through the vents and such. Goddamn, I had to pause the game to breathe and laugh nervously a few times.

Mannequin from Silent Hill 2. Its just two sets of mannequin legs top and bottom. Oddly disturbing somehow...
