In order of time spent
1. World of Warcraft
2. THPS
3. Twisted Metal

Now, no. 2 & 3 are old as fuck, but I'd say I probably gotten more play time out of them than most of the games published in recent years combined. Lets take like Deus Ex: HR, a 20h playthrough, and compare it to THPS, which I played for years, alone or with friends, so its hard for a game that barely keeps me entertained for a week end to compete with that.

1. Rome Total War
2. Medieval 2 Total War
3. Mass Effect 1, Victoria 2 or EU3 (can't decide which one has stolen more hours of my time.)

1) Fallout 3
2) Mass Effect
3) Dragon Age: Origins

So many hours lost between the three. Oh well. Who needs school, family and social encounters anyways?

Final Fantasy VIII I easily played over 500 hours of this game

Final Fantasy X a little less but still around 400 hours

and chances are the third is Planescape: Torment since I play it over and over again and still have loads of fun with it

Pokemon Red
Super Smash Bros Melee
Warcraft III

I'd say it's those three

I know Tales of Symphonia is the number one. After that though, it could be any of the following:

Pokemon Soulsilver
Resident Evil 4
Dead Space 2
Resident Evil 5
Marvel vs Capcom 3
Legend of Zelda: Majora's Mask
Rick Band 2
Super Smash Bros. Melee

Honestly, I have no idea where which game stands.

1. Counter-Strike. (from 1.3). Steam clock for CS:S is 1600+ hours and it started after it launched.

Other games that I have played lots and lots:
Elasto Mania

Probably these three, though I don't know in which order:

- Counterstrike: Source/Unreal Tournament 99, not sure which one as I didn't count my hours when I played those games.

- World of Warcraft with about 900-1000 hours, though I'm not playing any more. Good times were had though.

- Rollercoaster Tycoon/Simcity 3000

Though there's also the possibility of Super Smash Bros. Melee belonging in there, or Team Fortress 2. I really have no idea.

I would hazard a guess at...

Tibia though almost all the time was spent socializing.
Heroes of Might and Magic III, a game which a lot of people here seem to have lost many hours to.
Pokemon Gold back in the day. I think I had 350 hours logged, and 241 Pokemanz caught. Including a legitimate Celebi and Mew. Then the battery in my cartridge died, and my save with it. Aww.

Captcha: "be my friend". The captcha server is getting sentient and lonely. Uh oh.

3. Civilization 4. Over 1000+ hours.
2. Team Fortress 2. 5406 hours.
1. Diablo 2. Over 12000+ hours. (played it from 01 when its expansion came out until 2010 for 6 - 12 hours a day 6 days a week and since 2010 though not as much still play it for a few hours a month)
I don't mean to depress you, but according to this book (somewhat paraphrased): http://en.wikipedia.org/wiki/Outliers_%28book%29 you can be a world-class anything if you practice it for 10,000 hours.

You could've been a world famous spork maker by now! Assuming you're not, that is.

Pokemon Emerald at over 200 hours, Fallout 3 at over 90 hours, The Legend of Zelda: The Wind Waker at over 70 hours (multiple playthroughs, almost all items collected)

It feels weird writing this because I don't think I'll ever get close to those numbers again with most games, just due to the fact that I play most games in demo form and then never pick them up again.
