What are the most horrendous ways that you could imagine dying?

Being surgically disassembled over a period of many hours, and being made to watch as it happens. Or maybe, being crushed between two giant flat stone blocks, very slowly. Or a massive botfly infestation. I could give a lot more if I tried.

I'll first state the obvious like burning alive, drowning/strangulation, and being torn apart/eaten alive.
But the worst one for me -my secret fear- is death by exploding toilet.
Nothing is worse than dying while at you're most vulnerable, and knowing that your body will be found with your pants down covered in filth.

Having your bones melt by osteotic injections of Hydrofluoric Acid while the nerves in your teeth are being pierced by tiny flanged screws, while one of those little hookfish crawl up your penis while it's erect, capped with having a backbrace slowly tightened by slow, continuous mechanical grinding so that you bend in half, while a very dull knife works it's way across your abdomen in fluid, repeated strokes, so that you slowly split in half.

Being set to drift in space, alone and without no hope of rescue. Watching your air meter tick down while drifting through cold, empty space by yourself, unable to even interact with your environment... that's a real feeling of helplessness. You can try to claw your way out of water or pound with your fists at walls trapping you, but what can you possibly do in a sea of nothingness? You, who are an insignificant speck amidst a larger theater of existence, completely at the mercy of a nature beyond your scope that you are incapable of understanding, and entirely dependent on a technology that is always at risk of failing before you even have the chance to give it input.
I think starving to death or dying of dehydration would be horrible. Imagine that you were stranded at sea in a life raft. Everyday you hope for rescue but it never comes. That would suck.
As for horribly but quick ways to die, being forced into a "2 girls 1 cup" scenario and asphyxiating on the sheer volume of poo being forced into your esophagus.
Fun fact: I used more clinical words as a defense against actually visualizing this scenario....

Anything that is slow, really. Like cancer. Sure they can dope you to the gills and you won't feel much, but that light is coming and there isn't anything you can do. That seems the most horrific way to go to me.
A lot of the gory ones listed here, well, I imagine shock would put you out before too long and that, to me, would be much better than waiting it out over a period of months or even years.

Oh! A better one. First, you're strapped to a vertical table and given a device with a button. Across the room from you, the person you love most (wife, boyfriend, w/e) is strapped to a similar table. When you press the button, high voltage current runs through your table, giving you a horrible shock, but when the button isn't pressed, the same shock runs through their table. You have to electrocute yourself to death or watch the other person die. And if you just let her die, you have to watch dogs eat her corpse. And then they eat you.
Well, the central concept is there (inflict horrible agony on yourself or it happens to loved ones), but it seems like it could use a little modifying. Anyone care to help?
