I have not, nor will I ever, play the ending to Mass Effect 3. Thanks to that, the ME series as a whole remains one of my favorites of all time. Sometimes it pays to have a story spoiled.
The best thing to do is play right until just before the final mission, PRETEND you did the final mission and stopped the Reapers, than play the Citadel DLC and have that be the ending of the trilogy.

OT, I never play evil characters in games that give you a choice. I just can't bring myself to be an asshole in a video game. I also do my best to get the best possible outcome, and will replay a game if I don't get the best ending (or at least one I'm satisfied with).

Oh, I just remembered another couple from Mass Effect:
1) I will never pick Morinth over Samara in her loyalty mission. I've said this before but I can't see that as anything other than stupid.
2) I can never not hug Tali after a certain point in her loyalty mission in 2.

Ignoring the ones already mentioned, I'll never:

- Nuke Kaidan or leave the salarian STG team behind (Mass Effect)
- Have Elliot executed (Fable III)
- Play as a guy (Mass Effect series)

Why do I feel like I'm forgetting a bunch of choices? Damn you all for going for the obvious ones!

Lets see, I never chose to nuke Megaton in Fallout 3. Even if Moira almost gets me killed working on her book, they're good people.

I never killed any of the little sisters in Bioshock 1 and 2. The rewards you get for sparing them are way more useful than any extra Adam.

I never killed Parthunax in Skrim. I don't care what The Blades think, he's a cool guy!

I never left the shock collar on Vette in SW:TOR, even as a Dark Side Sith Warrior. I always found her more useful as a henchman than as a slave.

Getting the Bad Ending in Persona 3, or the Accomplice Ending in Persona 4 Golden are two things I can never bring myself to do.
In Persona 3, seeing the resolve that the other characters have to fight Nyx, I can't in good conscience kill Ryoji.
And in Persona 4 Golden, I love the other characters way too much to betray them and side with Adachi.

There's one quest in Skyrim that I will never be able to do.

One of the optional quests requires you to sacrifice one of your followers. I'll never be able to play as a complete evil character, so this is a no-go
. If you bring a companion, trap them. Then when told to kill them, you instead attack the priests, your companion will likely die in the following melee.

It doesn't change much as you have to kill them anyway. Then I justify taking ebony mail as removing it from the hands of evil users.

In the first Deus Ex I never choose the ending for Tong. Plunge the world into a second dark age? I'm afraid I can't do that, Mr. Tong, doesn't sound like a very good idea to me.

In Mass Effect, I'm never going to side with the Salarians in ME 3. Seriously, that councilor gives you shit for all 3 games, disregards everything you bring him, and basically considers you to be less than pond scum. Then, he/she (I'm not sure it's gender actually), shows up via hologram, to threaten you to not reveal their secret about the Krogan, and offers you their help. Um, sorry fuckwit, you have burned every bridge with me from day 1, and I have exactly zero fucks to give about your problems. Where as the Krogan have been in the thick of this shit with me from the start. I'm siding with them, you can kiss my ass. Also, this is because of how awesome Mordin is, and I won't betray him, because he was funny in game 2, and badass in game 3. So yeah, the Salarian councilor can go eat a Krogan dick, and choke on all 4 testicles. xD

If I ever play Saints Row 3 again, I'll never choose
to leave Chauncy to die. Seriously, fuck that Luchadorian dude, I already defeated him by taking his mask and humiliating him. I don't have to also chase him down and kill him. And I'm certainly not going to let that happen at the cost of Chauncy!
Actually that was the Salarian Dalatrass, their leader as opposed to their councilor - indeed the councilor, while being a doubting Thomas, was more or less on your page by that point.

But I agree, fuck that *****. Curing the Genophage makes my tummy tingle in a good way and since I was never dumb enough to lose Wrex or Bakara, the Krogan future is bright.

Ah, right ok, the Dalatrass, thanks. Yeah, goes to show how much of an impact the Salarians made on me over the course of those games. Though to be fair, that was the developers fault, not having any real involvement with them for most of the game. But yeah, forget that lady and her problems. xD Krogan for Life! *shoulder flex thingy they do to look tough*

..wait, you mean Red Faction the video game series? Red Faction: Guerilla for example? You're implying that that reality is the same as the Saints Row reality? As awesome as that might sound, I can't buy it. The bad guys in Red Faction are just not over the top enough to be connected to the Saints. They don't have glow in the dark dick lasers mounted on their gunships, or dubstep guns, or, well pretty much anything else that is batshit crazy, to dominate the people of Red Faction. xD
At one point they more or less were in the same reality, though I'm sure that was before 4. Ultor was the bad guy in SR2 and the first RF game, and both SR2 and SR3 have allusions to the RF series. The series has drifted since then....

Hah! That's hilarious. I never played the first two Saints games.

If you liked 3 and 4, play 2. Just be aware that 2 is a lot darker then the other two and while there's still plenty of fun to be had(like missions where you drive around in a septic truck and spray pressurized liquid shit all over buildings, people and police cars because you can), there's some bits where both the boss and the other gangs do really nasty stuff to each other(dragging someone behind a car across half the city, burying someone alive, putting toxic waste in a tattoo needle).

Never played 1, though apparently Ultor was just a clothing store in that one, and in the 5 years between 1 and 2, grew to become an evil mega-corp that more or less ran Stillwater. Ultor doesn't really show up in 3, they're just mentioned as somehow being controlled or allied with the Saints by that point.

In Mass Effect, I have not - and will not - ever kill Wrex. He's just too much fun and, in most playthroughs, an integral part of my squad as the good ol' bullet-sponge.
In general, I do everything I can to keep the Bioware Assholes alive and liking me. Wrex, HK-47, Shale...I love those characters a lot and usually keep them as a permanent party member once I get them through to the end.

Speaking of Bioware RPGs (and any sci/Fantasy RPG in general) if I'm able to customize my party, I tend to keep humans out. If I'm allowed to make my own characters, I generally play as anything that isn't human. I have to live as a human every day of my life after all...in games I want to be a lizard-man or a cat-girl or a fair elf or something along those lines. It's too bad they're all still basically humanoid...I wanna be some kind of giant spider...

In Mass Effect I never kill off Kaidan on Virmire, mostly because I find him far more interesting than Ashley. Plus he's the only romance for male Shepard, in my opinion. Lady Shep gets Garrus.

Also, I always cure the genophage, because fuck the Salarian Dalatrass.

I'll do just about every option people here wouldn't do...

I haven't killed Parthurnax though. I like him better than the Blades and the Blades don't give me much.

I have no objections to offing my squad in ME2 but the series is too long for a lame run like that.

I will pick most of the evil options in games though. I do multiple runs just for that.

I've never taken the Scoia'tael path in any Witcher game where it has been an option, nor do I know anyone IRL who has done so. The Scoia'tael just seem real jerk-y, all things being equal.
Interestingly (havnt played 3 yet) but i have never not sided with the Scoia'tael as the other side seems too jerky for me.

Also in Mass Effect I have only ever once chosen to kill the Rachni and then felt so bad i immediately reloaded my previous save and let them go.
