Typically one step above normal. I like a small challenge still, but at 32 with 2 kids and 20 year old gaming hands.. not quick as they used to be.

I play games for game-play, not the challenge. I played for the challenge for a while, and beat games like Devil May Cry, Dark Souls, and Final Fantasy on their hardest settings. I even beat Contra, for crying out loud (Two lives left), but it wasn't FUN. If the game's not fun to play, it's not worth the effort of the hard/insane difficulties. Certain games, like Ghost Recon, are meant to be played on harder settings, and I adjust upwards accordingly. However, the vast majority of good games are just more fun on easy/normal mode.

I usually play through on normal, but it usually depends. I made my first run through The Witcher on the hardest difficulty because I wanted to make the game last a while. It turned the fight with the beast into pure hell, not the mention the fight with Javed. :p

It varies depending on how many difficulty choices there are and what the game is. Normally I choose the second easiest difficulty for a first run through and many use a higher difficulty later. I usually try to beat the campaign on every difficulty, but there are some that I won't even try like CoD. To hell with Veteran mode.

Normal, i want to have a positive experience from my games, if i start on hard without knowing how the game works i might get frustrated and just 'alt+f4' it. But once i completed it i crank it up to make things more challenging.

Depends on how the game handles its difficulty.

My philosophy in general is to make myself earn the end credits, without making the game frustrating.

So for me that's usually normal, or the next-to-hardest mode, unless I'm already well practiced in the franchise(I usually play kingdom hearts on proud for example).

So if on a scale of 1 to 5, 1 is the easiest and 5 is the hardest, I'll usually pick 4.

I also start on the default/normal difficulty because if the game is too relentless then I might get too frustrated and lose interest in a game I paid for just an hour or so ago. Never really played a game on a hard or insane difficult on the first go but I will change the difficulty as the game progresses depending on how easy/hard it is to me.

I tend to start FPS's on the hardest mode (barring ridiculous one-shot kill modes), because feeling threatened makes them more exciting. And those kinds of games are generally pretty forgiving of failure, with frequent checkpoints and health.
RPG's I generally do normal or easy, because hard tends to just make them really tedious unless you're a big fan of the game and want a challenge. I try third Person shooters/action games and RTS's on normal because the difficulty can potentially be all over the place and I like to test the water first.

Usually the hardest difficulty, I don't like playing games over unless I really liked them, and that extends the life of the game and is fun. Thats why i HATE unlockable difficulty levels DX

On most games I would start at the normal difficult and if I choose to play through them again, I do so at a higher difficulty.

Also, if I play games with shorter campaigns with more emphasis on the multiplayer, I tend to go with a hard difficulty to provide me more of a challenge and a better experience.

I used to play on easy but now i've it on normal becuase of
a) out of honour
b) i get better achiveemnts for it

so after playing gears 3 i check my achivements
"beat gears 3 campaign on normal or easy"
...

And I want to say that I do not find difficulty very fun. If I can't achieve my goal with relative ease, I get frustrated. That's the opposite to the desired result, which would be relaxation and escapism. Unless easy mode is ridiculously easy, like in Kingdom Hearts 2, or it actually ends up being more difficult in some ways, like in Tales of Graces where you learn abilities far, FAR slower than in any other difficulty due to lack of combat bonuses, I play easy.

I'll also play on harder difficulties if there is easy mode mockery [http://tvtropes.org/pmwiki/pmwiki.php/Main/EasyModeMockery] in place. I hate easy mode mockery, though. It's mean and inconsiderate.
