Normally, I start on Normal, because it's supposed to be the difficulty that gives the best balance to the game for fun.

If it's not, then I switch it accordingly. Or, if I'm doing a second run through, I'll turn up the difficulty, having learned the mechanics, strategies and AI behaviour.

Normal on my first play-through, then straight on to the hardest difficulty for any additional play-throughs.

Plain ol' boring normal. I feel like a wimp for selecting easy, but I can't really be bothered with hard. When I actually play something that's very difficult I rarely give up and enjoy the challenge, but it never seems like a good idea when I'm starting a game.

First run through is always normal. Afterwards, I amp it up to hard or insanity/suicide/whatever variation there is.

OP, there was a point in Catherine where I had to try my very hardest not to start the whole game over on easy. I agree with E=N, N=H, H=I. To anyone who didn't have a hard time with it at some point, well eff you!

I usually play on hard, or the difficulty between insane and normal/medium, unless the game is a bit harder than I expected or I'm unfamiliar with the game (tends not to happen, so many of the games I play are alike). This usually provides me with plenty of challenge, and I don't die so often that I feel like smashing my cheap shitty keyboard.

Max, although there are times when these cover based shooters are so obnoxious; if there are enemies alive and you're moving you're just dead. It's as though the game were saying "Silly player, didn't you know the whack-a-mole phase was underway? No moving until the big blinky arrow appears!"

I always start with the normal or default difficulty. I like to have a challenge, but not something overly frustrating. Maybe if I replay the game I might try a harder difficulty since I will have had a feel for the game.
Same here. The exception for me is Skyrim, which is incredibly easy no matter what difficulty I play it on (granted, I do tend to make myself overpowered gear, but it's still legit).

Also, I sometimes like to run through a game on easy just for kicks. For example, a CoD campaign on easy allows you to pretty much knife everyone. As with Halo campaigns. So entertaining. :)

As hard as a turd that's been sitting out in the sun for 6 weeks.

Starting a game on hard and actually being able to play it well after some time means the game is very good at teaching you how to play and the mechanics are polished.

Normal if I'm new to the game/series/genre, but for, say, my seven hundred and twenty-third playthrough of Mass Effect, anything less than max difficulty won't cut it.

I always play my games on the normal or middle difficulty level. Anything less would seem a disservice to myself and any more could potentially frustrate me.

I usually set it to the one right above normal (so even for games that go "easy...normal...hard...insane...nightmare...death...hell" I'll still just choose hard), and I'll change it to normal if necessary, but usually the games I play aren't that hard.

I play through it my fist time on normal (no constant rage, able to concentrate on the story, look forward to parts without dread), then I play on the highest difficulty setting (achivments, rewards, bragging rights, etc.)

I start default, then adjust from there...unless it's Devil My Cry, then it's easy because I am the worst player alive at that game.
