I usually use names that I've heard recently for games like Fallout, otherwise I use my own. If I'm playing and Elder Scrolls game then I'll make it a lore friendly name using the wikia and mashing up a first and last name from previous games.

Phil.

It's always Phil. No matter what game, Mass Effect, KotOR, Fallout, Elder Scrolls, Pokemon, I will always be Phil.

I think it started in Knights of the Old Republic, when I was trying to think of a name. The names were incredibly stupid when I picked a random name, so I decided to choose the most ridiculously normal name I could think of. It was hilarious because it didn't fit at all with the universe. And so I decided to use it forever.

Same character, too. Scrawny little white guy with a beard, fairly smart, fairly nice, likes to crack a joke here and there, has a strange obsession with swords (mostly katanas).

I usually base the name on the play style of my character.
Case in point, my tank character in Dark Souls is called Murum Invictum, which is Latin for invincible wall.
I don't limit the language to Latin; I'll usually fluctuate to German, Japanese and Greek.

I usually go with Mordread, was the first online char I ever made, back in the EverQuest days. That or sometimes Agamon.

(My nick "MordreadRN" is the char I made back then and I played on Rodcet Nife :) I used it once to register on some board and it just stuck with me.)

Doug.

I like to keep it lighthearted, so as I save the kingdom or slaughter the kingdom, I like to imagine them telling tales of Doug the Glorious or Doug the Terrible for generations.

"... then a great and mighty man arrived. He was dragonborn! He was... Doug!"

I just tend to name them whatever I think of at the time. Quite a lot of time though, I just either use my real name or use some weird crazy variation of it to suit the situation.

Depending on how much space I get, my females are usually named Juana Killalot, Stealthalot or something completely unoriginal. If it's male I stole the name Goblecoq from.. someone. Croshaw? I honestly cant remember but it was the name they used in Psychonauts and it made me giggle like a little schoolgirl.

Just a side note. If this is in an MMO I'm not so crass, I'll use the surname Stormhawke with one of my usual character first names, Fo.. HEEEEY! no stealing my names, I see what you're doing you're trying to name camp me in GW2!
lol my thoughts exactly!
but for non mmo games i go with draco most times.

If I'm not to arsed about the lore of the game in question, ShadowKrai (I rage if I can't capitalize the K)
If it's an Elf, Thalesin
If it's a rogue/necromancer/other shady thing, Noktis
If it's an Orc, usually Drakai

I tend not to be too bothered with Lore, but if it seems like an interesting one, I'll try to make my name using the same rules of the others of my chosen race's name.

As for guild wars 2, if you see ShadowKrai the human necromancer, you rolled on the same server as me.

I often use Tasartir as my character's name from a D&D campaign I have been playing for a long time.

In the case of Mass Effect, I named my main character Shepard Shepard so that everyone would talk to me on a first-name basis.

River. I always use the name River. Though it's usually the last/family name.

I like to pretend that all of my RPG and Dungeons&Dragons characters are alternate universe versions of each other. Or if they're in the same universe, related.

It all depends on the game and character I'm playing as.

Most games I've gone with Soren[footnote]Games that I've played as a Soren include Dragon Age: Origins, Pokemon White, and Golden Sun: Dark Dawn.[/footnote] because I'm dull (although for Fallout: New Vegas I did go with 'Soren the 7550th' to spice things up a bit).

For Mass Effect my Shepard is named Roslyn (I like the name, and I thought it'd be a befitting name for a tough character), my elf playthrough for DA:O I named Sandraudiga (she who dyes the sand red) since that was going to be my evil/bad playthrough (ok, so my elf was a guy, but the name was too good to pass up), In Pokemon Silver and its remake I named my character Sam.

My number 1 character is Fallout 3 and Fallout: New Vegas have the same name, Maloney. A melee centered sneak thief. I just like the name Maloney.

Wasn't it Yahtzee who recommended calling yourself "I Say", so that every other character sounded like Foghorn Leghorn?

For males I usually go with Faren which means Adventurous or my name depending on the game and for a girl I either go with Lilium, the name of the theme from Elfen Lied, or Jade.

I usually name my character after myself, but in it's original gaelic spelling (Ciarán Ó Conaill). It's sufficiently strange enough to fit in a fantasy setting, but normal enough to fit in any other setting. For females I just use Ciará Ó Conaill. I'll also just use the first name if I'm lazy or it doesn't let me fit a last name.
