Ninja Gaiden Black- three times, was working on a fourth when Mass Effect came out.
Mass Effect- Three times, was working on a fourth when I stopped caring.
Gears of War- At least three times.
Fallout 3-2 or 3 times
Halo 1- several dozen. At one time it was all I would play.
MGS 3- at least three times
Soul Calibur 2- four-five
and 3- three-four
Mechwarriors: Mercenary- about three times
Warcraft-Reign of Chaos and Frozen Throne both about 5 times each.
Starcraft- Regular and Brood War combined more than 20 times.

I have a whole bunch more PS2 titles that I've played through a couple times.

I've probably played through Half-Life more than 75 since it's initial release. I just really really like that game. The only other thing that comes close is the real Call of Duty 1 on the PC which is probably close to 10 I'd wager.

Timesplitters: Future Perfect about a billion times. I'm not sure how many times I played the whole game (over 20, I bet), but I played the first 3 or so levels a good 40 times.
