God of war ghost of sparta
I played it long ago but I remember it because theirs this move the boss has that can be basicly a 1 hit kill and I spent 20 minutes trying to kill her then while shes on her last pixel of health she kills me with that move I threw my psp (luckly onto my couch) and yelled at the top of my lungs for 5 minutes

In this console generation:

inFamous - The Final boss was pretty tough on hard difficulty.
CoD:MW2 - I hated the favela, it seemed simply unfair on the hardest difficulty.
Bioshock - I did the whole game without dying. Big Daddy fights were sometimes rough, and the beginning few levels were a real pain too. I think the final boss was a bit tough as well.
Mass Effect 2 - Horizon. Oh my god Horizon.

I am pretty sure there are others, but my mind is feeling most blank at the moment.

Special mention goes to VVVVVV, despite not having difficulty settings. It is just hard.

75% of the games ive ever owned, its a habit to beat it on the most difficult setting as it usually results in awesome fights

Arkham Asylum and Uncharted 2 are the only ones I can think of at the minute. Poison Ivy was a nightmare, and I think the train level was the hardest in U2.

Bulletstorm
Halo: Reach
Crysis 2
Timesplitters: Future Perfect
Gears of War
Splinter Cell: Conviction
Resident Evil 5 (although this one was on co-op)
Halo 3: ODST (on co-op)
Star Wars: The Force Unleashed II

I can't remember what levels were the hardest or what most of the levels were even called for that matter. Star Wars: TFU2 I'm actually on the last mission but I got bored and stopped playing because I'd already completed the game 3 times. I counted it because while I haven't done the last bit, I could if I could be arsed to.

All the CoDs except one mission in CoD2. You know, the one where you start of in a small alley and enemy troops with panzerschrecks and STG44s shoot from on top of the buildings and they obliterate you 2 secs after the mission starts. Also Halo Reach solo on legendary, Gears 1 and 2 on Insane, Star Wars:TFU on Sith Master, basically most games I own I beat on the hardest difficulty. If any of you want a challenge, I suggest Brothers in Arms: Hells Highway on the hardest difficulty.

Halo: Reach - the final full campaign mission was a total nightmare. That final elite was one tenacious individual.
Gears of War 2
Mass Effect 2
Bayonetta - that was probably one of the most gruelling - don't think I ever want to experience that final Jeanne fight on Non-stop Infinite Climax mode ever again. Nasty.
