OP: I'd have to say the "You Are Dead" screen on God of War 3. Hadn't played in ages and had lost my skills (had beaten the other two on their hardest difficulties back in the day), so even playing GoW3 on Titan gave me a lot of deaths. So yeah seeing that screen a lot was very annoying.

Pretty much anything if it's repeated enough, but I find any game that has you go and search for rare random drops in high quantities to be a really poor design choice in and of itself.
Also, Pokemon games. You're in an area that has pokemon too low-level to be of any use to you, but you have no repels. The fuckers just keep popping up and wasting your time. Don't get me wrong, I like the pokemon games, but bloody hell!
