Anything that isn't instant. Getting a limb cut off and bleeding out is usually painless, as your body goes into shock.

Drowning isn't nearly a bad of a death as it is a feeling. The feeling only lasts how long it takes for you to die, but there is a gas that contains enough oxygen to keep you alive but the feeling of breathing it in is identical to drowning.

Sand papered to death. Toes to head. Slowly, but surely, you will be sanded to the bone, tearing away tinytiny chunks of your skin and nerves until your femurs are smooth as melting ice.

*shiver* I have nightmares like that.

One of the drugs we have a placement, Suxamethonium, that would be a nasty one. It's normally used to paralyse people once asleep to intubate them and stop the muscles moving during the op.

Giving it to someone awake would be hell on earth for them.

Can't breathe, can't talk, can't move. Just left there fully conscious to slowly suffocate.

(Also, Sux is the only muscle relaxant used in hospitals that has no reversal, so rules that option out).

Ever burnt yourself on a hot cooking plate?
Imagine a room with said plate as floor and it will slowly increase in heat over time.
No objects in the room, only a big floor, walls with no windows and some heat resistant lights. And roof, obviously.

As it slowly increase in heat, your body want to move away, but anywhere you go, it's the same.
In the end the muscles in your feet can't take it anymore and you might drop down on your knees or your back. Slowly, being boiled alive.

Of course this is more of a mad man trap, not a realistic death senario.
For a more realistic senario, I would say being chased by a shark and devoured. But that might be more of a specific thing for me, due to my fear of deep water and what you can't see.

Back in the days they used to tie people to the front of a ship, near the water's edge. What happened was that you only died after two to four weeks, mostly out of starvation or sleep deprivation. While you were alive, however, you were not so happy. The crushing weight of wave after wave eventually shattering your bones while constantly pushing / moving you over the barnacles that already lived on the hull which you are tied on to.

A close second is dying in a fire.

Having a glass tube shoved up your urethra and then someone smashing down with a hammer, which will lead to slow, painful death by bleeding out.

OK, the coffin torture sounds pretty bad.
Locked in a steel cage for days so you can't move while animals peck and gnaw at you. I imagine the eyes would be the first things to go... followed by any exposed digits/fleshy parts.
