All the stuff involving the Suburban Redevelopment Fund was dragged out a bit too long for my liking and some of the people involved with it got away with it too! Justice wasn't served, that's what annoyed me. Surely the exploitation of everyone on the SRF would have done the trick?
Noir books and films never end well. The main character almost always dies and justice is never served. You're meant to love to hate noir for that very reason. I'm pretty sure everyone was meant to scream at the game when Roy spoke at your funeral.
When he went up to make the speech, and I saw the Superintendent up there (the one who was part of the SRF) I was like 'NO! SOMEONE SAY SOMETHING!' but they didn't. It was sad for that reason, as Cole died for nothing, it seems. That's why I found the ending weird, that's never really happened in any game I've played...apart from maybe Roach and Ghost's deaths in MW2

The ending to Y-The Last Man was a bit of a let down...but then again so is life, and I think that was the point.
Any game that sacrifices fun to make an artistic statement is completely missing the point of being a game.

OT: Oblivion. The ending had such a build up, and included the daedric paradise that was extremely interesting, and then Martin killed the main enemy through Deus Ex Machina. And afterwards no-one cares about how you single-handedly closed the oblivion gate.

Any Obvious set up to a sequel(Bad Company 2 *cough*) or anything that ends in the middle of a heated battle.

Berserk, god dammit. >.< I nearly put a chainaxe through my TV...
The Anime only Covers a small part of the whole Manga, which by now is nearly on 300 Chapters and is nowhere near the End just yet. So if you want more, read it!

OT:
Most recent terrible ending was DA2. The game was good, the story kinda worked, the ending just sucked. You pick a side and then fuck over the leaders of both sides anyway! And then when it actually gets good, it ends. You dont even get a preview of what has happened aside from "Shit hit the Fan", nothing even remotely specific. Hell you dont even learn if the Hawke is still alive (i.e. you play as him/her in DA3)

To be fair, that didn't really end, that was just cancelled.

Anything with pure, raw, deus ex machina.

Not Chekov's gun, not fridge brilliance.

Avatar: The Last Airbender comes to mind.
You mean the whole spirit bending spiel?
But where the series ended was totally whack, so I still think that's fair.

Harry Potter's ending:
I wanted him to die!
What else? Erm... Bridge to Terrabithia? Ok I know the entire film is crap, but the ending just sucked.
And generally anything that ends with an 'it was all just a dream' type scenario.
The book was better. I cried. I love that book.

Edit: Although the book is better in both examples I was referring to Terabithia.

Y the last man is a comic book not a game.

need I mention Killzone 3's ending. Nothing gets tied up, except that you just commited mass genocide, and of course with a friggin cliffhanger after the credits

The Mist.

I felt like that film flipped me the bird.... like really, they (if you've seen it you know who I'm talking about) showed up right after he (again if you've seen it you know what I'm talking about) did that?
I loved that ending! It totally screws the guy over, and that moment of realisation when you think 'if only he'd waited a few seconds!' makes you feel sad for him.

OT: Why has no one said F.E.A.R 2? That horsesh*t of an ending resolved NOTHING!!

And then it was all a dream.

And then good kicked evil's ass and everyone lived happily ever after.

And then everyone lived happily ever after.

Seriously; happy endings are great and all, but sometimes they just aren't realistic.

Also, Dungeon Keeper's ending was irritating because it ended. Same with a few other games. But mostly Dungeon Keeper.

Fallout 3.

You're a robot...radiation doesn't affect you!!!!

Also the ending slideshow didn't give enough closure.

How did I miss that ending?
You need to get all 65 weapons, level them all to level 4, get all of the companions and beat certain levels in certain ways. It's an absolute pain in the ass to do, and the final boss fight and final cutscene are so fucking bad that it's just not worth it.

Mass Effect 2. And not for the reasons one might think. It's because Garrus died in my first playthrough. Because of my own damn fault. >

Also...

Might I just throw these two little words out there? Broken Steel~

Fallout: New Vegas' ending. No, Obsidian, I do not value a fucking slideshow over the possibility of hundreds more hours of gameplay.

What's that? You wanted to add a really rich sense of reactivity to the ending? You know how else you can do that? By adding dynamic gameplay.
