Having some relationship between two characters that I don't care about. Actually, having any sort of relationship in every game. That kinda gets old fast.

Overused suggests that it's used too much. Widely used just means it's used a lot.

Allies that don't help in any real way. Like when you charge into battle with a bunch of nameless soldiers/knights/space marines who just crouch behind barrels and occasionally fire bullets blindly in the general direction of the enemy. I know that if they actually were useful in a fight, they would just kill everything for you, thereby eliminating the challenge of the game, but at least don't make it so obvious that they don't do anything.

I hate it when designers feel the need to shoehorn a cheesy love interest into everything.
