Dressed my character in Fable 2 all in black and got an achievement called 'The Goth'... very funny!

Also the achievement for killing an enemy with a toilet in HL2 was genius!
You beat me to it!

Another couple of World of Warcraft ones.

"Representin'" earned when you by your Guilds tabard obviously thought up when blizzard employees got bored and played a lot of GTA while listening to hardcore rap for a couple of days.

Next is when your fishing you have a chance to get a book snagged on your hook which will "teach you the ways of fishing passed down for generations" or in normal English to locate schools of fish on your minimap.

Achievement unlocked "Fish Have No Legs"

Probably from Half-Life 2: "Defiant" and "Submissive", awarded for either putting a dropped tin can into a trash can, or hitting a cop in the face with it.

Targeted Advertising for pinning a Combine to a Billboard (only happens in one place) and Flushed (Kill something with a toilet) were funny, but Flushed was irritating. There's a ton of toilets in the game, but only a couple you can pick up and fling.

While my wife was playing over the weekend, I unlocked the "Gotta Have More Cowbell" achievement in Lips. You have to hit the cowbell sound effect more than 50 times.

And I was just trying to mess her up :p

In the first level of Earthworm Jim when you launched the cow and it wasn't mentioned ever again throughout the game.

Getting the chain reaction one in GTA 4 by blowing up a bus in the middle of a busy intersection. Although on COD 4, I saw a claymore through two walls with bomb squad, and I got "How the...?!" by killing a guy with it as he charged past the claymore into the building. I was stuck sitting there going "What the f..." for like 20 minutes. My friends were going crazy.

Well, I'm pretty sure there are weirder ones I have, and I'll have a look later, but the strangest achievement that I can remember is "Press Start to Play" from "The Simpsons Game" which you got for pressing start on the title screen. 5 Gamerscore, and even the achievement description says "Easiest achievement, ever..."

EDIT: That was weird.
Someone just beat me to it. Oh well.

Purple Heart on CoD World at War. It's basically 5 points for getting killed a bunch. Which wasn't hard to get because the campaign on that game had more bugs than my first apartment and the AI consisted of a thousand Germans raining grenades down on your face. I got it during the ill-conceived sniper mission where you're dueling with another sniper. God that mission pissed me off.

Dead Rising wins simply for the sheer number of odd achievements that give you something to do...Raining Zombies, Frank The Pimp, the novelty mask one...

Two from Mirror's Edge were interesting..."Hey, It's-a me!" where you do the stomp attack, and the one where you trigger the flip-off gesture. Some of the move-combo ones were interesting too, like "...aaaand safe!" where you have to combo together a wallrun, coil-jump over an obstacle, and a skill roll, or possibly "May I have this dance?" which is essentially a double wallkick.

CoD 5 had an interesting 5-pointer which you get after dying multiple times at the same checkpoint during campaign, called the Purple Heart. Easiest to get during Veteran, lol. Some of the multiplayer challenges are cool and/or funny, like "What The ?" where you kill someone by shooting a land mine through a wall, "You Talkin' To Me?" where you survive a tank round on foot and quickly destroy the tank, or "Oh No You Don't" when you destroy a mine immediately after it's planted and kill the planter in the process. And then there's always "Cruelty" where you kill someone, pick up his weapon, and kill him again with his own weapon.

Probably already been mentioned but, the one where you broke the teleport machine on Half-Life2; I didn't even mean to do it.
And the 2pointer achievement getting you 2Gs, I spent ages! trying to figure out where the hell the uneven number came from because everything seemed to be ending in 5s and 0s, for some time I thought it was like interest on the good player reviews people would sumbit about you.
Now I know how silly that was.

"Little Rocket Man" -Half Life 2: Episode 2

I didn't carry the gnome to earn the achievement, I just though it would be a waste to leave a perfectly intact garden gnome lying around. Then I saw the achievement and put the little guy in the rocket. At least Lamaar had some company :)

Yeah there are some impossible achievements though (Well, almost)
like on shadowrun with 'God-Like' where you have to kill the whole enemy team on your own without taking any bullets and without any of your team firing one single shot at an enemy, and all this has to be done in the first round. which if you've played shadowrun, you know that is nearly impossible; not really the not taking bullets part (although that is hard) its more the teamates not inflicting damage part. So if you've seen someone with that achievement you know that either they did it in a private match, or their team when they did it was full of retards.
