unskippable cutscenes, boring/incredibly repetitive play, inconsistent mood, and leaving plot issues unresolved come immediately to mind.

Take control away from the player in what should obviously be a playable segment.
This.

To give an example...

Spoilers for Fallout 3!

...the scene with the Enclave invading Project Purity. If the plot can't handle allowing you to have your character react realistically, the plot needs to be reworked.

Getting a pirated copy without knowing and become subject to DRM countermeasures such as the game restarting on you on the final boss fight... intentionally.

Or if you're not that careless... escort missions when you have to protect the same guy both to his destination and back WITHOUT RECOVERING HEALTH AT SAID DESTINATION! He even got an apple pie...

mute protagonist.
Ohh, oh oh, what about this one:

After all these years all she had to say is "ADAM ADAM ADAM! I'll ignore my lifesaving suit powers just for you Adam! I'm a useless woman, WAAA!" it's like the developers were making a cutting allegory of sexism, what if feels like to be a broken subjugated woman... at the cost of making the game unbearable.

I just think it's really boring.
Have you considered maybe watching movies instead? Maybe gaming isn't for you, gaming for narrative at least.

Stop expecting video games to be like motion pictures. It's like complaining about something like a montage cutting randomly all over the place, when that is an absolute staple of film-making.

If you are getting bored in Half Life because Gordon Freeman isn't constantly monologuing to himself... oh I am just terrified some marketing moron will chose you for some "focus group" and say "hurr, protagonists must talk, screw decades of gaming convention and artistic intent, this guy want 2 hear hiz voice"

Artist: "but what would that add to or distract from the immersion, the interactivity, the involvement"

Marketer: "Herp, don't care, audience getting bored. Must sacrifice EVERYTHING to stop them getting bored... ever. They probably don't care about realtionships with characters, jsut want to hear 'stuff'."

Be a singleplayer game that ADDS MULTIPLAYER.
Hey! Don't bring Mass effect 3 into this!
I wasn't... in a way that might be a fine way to do it, though the whole "social network galactic war" thing is dumb in my opinion.

What I mean is adding multiplayer rather than focusing on the singleplayer.

Too many games are doing that now. Mass Effect is and it's likely it will fail but maybe not, still the trend has failed in MY OPINION and game-value rating system, for many many years.

The worst thing a game can do generally is fail to do what it is supposed to (generally entertain) but keep you playing anyway through reinforcement tricks so you end up hating it but want to beat it basically making you addicted (mildly or otherwise) to it through hollow awards that seem to increase but in actuality are completely meaningless.

If the game is supposed to be educational (like teach you a language or fly a plane or learn about a particular crisis for instance) and fails at that well thats pretty poor but if it convinces or tricks you into believing what you are learning is right well that could be a big problem.

Not having auto-save when it is clearly the most sensible thing they could do. Or if they have save points that are hours of gameplay away from each other.

Regenerating health
Regenerating health is wonderful when it's necessary.
You can't criticize regenerating health without looking at the problems of medkits and health stations as well.

This is only a problem for me if I don't have the ability to save myself. Otherwise, I pretty much save a game every 5 minutes or before going into new areas.

Personally, and other people have said this as well, the worst thing a game can do is be boring. The second worst for me would be large difficulty curves, that aren't warranted.

fail to be fun.

or crash behind a screen that makes you think its still going on or you're just waiting.
