Pool of Radiance: Ruins of Myth Drannor
Released during the last run of good D& D video games, this was planned to use 2nd edition rules but changed the new 3rd edition rules during development.
The result was an extraordinarily buggy game, which would not itself qualify the game for worst ever, except that one of the bugs would uninstall the PC's system files when the game was uninstalled. Which pretty much qualifies it as malware under the guise of what was then considered a AAA game.

Shirley Muldowney's Top Fuel Challenge. Tune-up car. Drive car for three to ten seconds. Have car suffer catastrophic and frequently inexplicable failure. Repair. Repeat until you run out of money. Lose.

And if I may have a word, since I keep feeling the need to belabor this point: They made way too many copies of E.T., and that contributed to the demise of home video gaming for a time. But the game itself really isn't that bad, mostly suffering from trying to allow the player to access about a dozen powers through the use of a single eight-direction joystick with a single button. There were far worse games on the 2600, and certainly far worse games across all systems ever.

Hey, E.T. didn't even try to destroy the system that was playing it, which is more than some games can say.
