Angersome.
Particularly since it's meant to be veteran mode.

Other than things not meant to be in the game liuke glitches and crashing, I think having to pause to do something is annoying. Like inventory. In Diablo 2, you check your inventory and things keep happening. I like that. Makes htings more hectic if you can't pause to heal or whatever.

It's one of the things that's really stopping me getting through Mass Effect. Every time I go to do an action, everything freezes. It makes the game less exciting, because I know that if I get surprised by something, I can just pause and figure out how to deal with it. Also, quite often you need the powers the wheel gives you to deal with stuff, so if you don't pause it, you won't be able to cope, but if you do, it ruins the hectic battle or whatever.

Bad animation. I can't believe nobody's said that yet. So many games suffer from this, especially Bethesda games. It kills immersion for me.

In that case, don't play DragonAge. Plenty of pauses per fight.

Also, your comment on inventories is a valid one, which is why I liked the inventory in Dead Space.

In-game, pop-in or serious clipping. Once I see it, I can't un-see it.

Out-of-game, definitely phone calls or knocks at the door.
Solution: headphones and ignore door and phone, works wonders for me.
I <3 my surround sound, though. Used to use headphones late at night, though, as to not offend my neighbors!

QTE EVENTS. Here I'm ready for some action, my hands ready on the control, watching the screen, thinking of move, and then BOOM! Suddenly QTE ruins all the fucking tension. QTE require no fucking sill whatsoever, and developers only put them there cause they ran out of programming budget.

Bugs, and glitches mostly graphical. This became glaringly apparent to me in Fallout 3. I would be walking along and all of the sudden there is a rock just floating there.

Odd animation. Yet again Fallout 3 but worse in Oblivion. The way the characters stand when you talk to them just brings me right out of it.

Also the rag dolls in Demon's Souls, I love the environment but the rag dolls just kill the immersion for me.

When I'm told how to 'double jump', especially in realistic games. This happened while I was playing Dante's Inferno during the tutorial, and this was way before I had even gotten to hell. I was slaughtering Saladin one moment and the next a window pops up explaining how to jump mid-air. I found myself wondering 'Where the original Crusaders able to jump twice? Was gravity not invented yet?'
