Having sound can also cause that, when it's very quite, there are many doorways, the game has bad sound direction xD

The scariest games are the ones where nothing happens. If the environment is constructed well enough and mixed with appropriate sound and music, a game should be able to create enough tension that the slightest background noise or movement sets you off without the need for "boo, gotcha" jump moments. Silent Hill is one of few games which achieved this. A notable section of the second game is when you find yourself in a large courtyard with a guillotine in the centre and you can constantly hear the sound of a horse galloping around you. You never see the damn thing and absolutely nothing happens within this room, but I still consider it the scariest part of any game I've played.

Haha, seriously though just flickering lights and that moment where you think you just saw something run across but you aren't sure. Until you have forgotten about it and then BAM "Haha! I knew I saw something! Aww shit...."

system shock 2 and the cradle level in Thief (can't remember the number/subtitle) have ensured i find no game truly scary ever again... dead space made me jump a couple of times though.

Its atmosphere and enemy design for the most part, and a damn good/appropriate soundtrack

A game needs to be unexpectedly scary to scare me.

With games like Dead Space or the (older) Resident Evils, you KNOW it is horror. "Oh, a window! Boy I sure hope nothing bursts through it to try and kill me!" You are more guarded against the fear when you know to expect it.

STALKER avoids this quite well, as I just figured that it was an FPS pseudo-RPG. When I started, however, I found that the enemies were numerous, the environment could always kill me in an instant if I was careless, and ammo was in short supply. Then I got to Strelok's stash. Since I was not expecting the game to be so creepy, so I ended up getting really scared.
