Real events and bad games/movies/books.
Good and popular games/movies/books should get spoiler alerts as not everyone gets around to them all that quick, and ruining it for someone isn't worth the few seconds you save by not adding a spoiler tag/warning.
This -may- not be believed, because it is hardly imaginable. But there are real and actual people that have been born after the massive Star Wars-hype (for instance). Of that group there are also people that haven't even seen it. *gasp* Sooo, do those people not deserve a story with an unforeseen twist? Or do those people not care enough about the movies for it? Surely if they cared, they must've seen it as soon as they would be able to read/post on forums?
And, yes. There are also people out there that don't care about spoilers, they can click on the spoiler tags.
So, no matter how old, widespread or well-known something is, please just take the effort.

Anything that's based on real events shouldn't need a spoiler.
It shouldn't be a spoiler that at the end of Titanic, the boat sinks.
It shouldn't be a spoiler that at the end of Lincoln, he gets shot in the head.
There's no chance of people not knowing about a historic event before seeing a movie/reading a book about it? Maybe not even knowing it was based on real events until this shows up in the credits?
Apologies for the (negative) sarcasm, I get a bit funny about spoilers.... (Don't watch trailers/read movie summaries because of it.)
In the next Mario game, Bowser will kidnap Princess Peach and Mario will be dispatched to save her. Using various power-ups while traveling through varied worlds and levels. I know. Huge spoiler.
And in the next Zelda game, Link will have to save her from Ganon using the Master Sword.
In the next God of War game, Kratos will kill a lot of people.
In the next Halo game, Master chief will only be able to hold two weapons at a time.

Generally speaking, I'd rather avoid spoilers as much as possible. I think it's fairly safe to spoil something that happened in the first five minutes or sets up the general theme of the show, movie, or game. But you would always be wise to avoid spoilers as much as possible.
Unless we're talking about a classic or well known older film, game, or movie. Then, it's kinda expected. You can't exactly talk about Casablanca without talking about plot points. Or Final Fantasy 7 without bringing up some of the major moments.
Disagree. I'm still annoyed that the ending of Public Enemy was wrecked because someone carelessly spoiled someone's fate. Not everybody knows the story. Not everybody needs to be told the obvious before going in- no matter HOW obvious it is.
Unless you're on a forum specifically dedicated to the discussion and appreciation of X's nuances, where X is any game/film/book, ANYTHING can be a spoiler, and thus, should not be spoiled.

None. There is absolutely no excuse to not use spoiler alerts. Something people on this forum really need to fucking learn how to do.
Not to mention how to do them correctly. When Season 3 of Game of Thrones was airing and everyone wanted to talk about it on this forum. Hardly anyone bothered to fucking specify whether the spoiler tag was for the show or the book, or which book in the series it was. Clearly state what you're spoiling before you use the tags, use the fucking tags, done. That's all you have to do.
See this? This is the post of someone who's had something spoiled because someone else couldn't give a damn or was too ignorant to realise what they were saying was a spoiler. My sympathies, friend, I've been there too, and I wholeheartedly agree with your post.

I think it depends on the game, film or book.
For example, if you spoil the twist at the end of Silent Hill 2, you've sort of ruined the experience for another person because up until the twist as far as you know you're looking for your suddenly alive again wife in Silent Hill.
Whereas if anybody seriously puts Spoilers for say, The Empire Strikes Back, they really underestimate the common knowledge of the fact that Luke is Vader's son, hell it's even revealed/heavily focused on in Episode III. I think it depends on how old and how popular the title is, or how much said spoiler will effect the plot.
Does it really hurt to say that the Elites get betrayed by the Prophets in Halo 2? Is it ok to reveal that Atlas turns out to not be who he seems in BioShock?
And in a few short sentences, you've managed to spoil several big things.
I, for one, had not played Halo, nor Silent Hill 2.
Do not assume that just because something has a large enough acclaim and following that you can't spoil it, because not everyone has played/read/watched it. Just because they haven't experienced it yet doesn't necessarily mean they won't ever.
On this site alone I've had several the following spoiled:
Batman Arkham City's ending
Both the good and bad ending of Infamous
So many Game Of Thrones spoilers (because people can't be fucked to mention it's the book they're spoiling in a thread about the show)
And while I played Bioshock, I've seen it been spoiled on this site so many times anyway.
Ugh, I get you, man.
I've been on the receiving end of quite a few Game of Thrones spoilers, too. Now I know things that I didn't want to know, at least until the next season starts.
I'd say that you have to reach absolute cultural saturation before it's okay to be outside of spoiler tags. Films usually become okay quicker than games do, because game storylines aren't as well known/ easier to avoid reading about. I played Bioshock 1 only the year before last, and hadn't had it spoiled for me.

Star Wars: The Empire Strikes back doesn't. Because everybody should already know that Vader is Luke's dad.
None. Because the point of spoilers is a courtesy to other people and not the media in question. The show/movie etc. that you spoil doesn't give a shit if you spoil it. If you spoil something for me (regardless of your opinion of the quality or time elapsed since release) you're an ass and I'll punch you in the face if its in person. If its online I will just retaliate by sending you the end to everything I've ever read or watched.
