VLC, it's always played whatever I've thrown at it and is easy to use, reliable and just good.

Foobar2000, it's lightweight and very customizable. If you're going to try it out I suggest trying to dig up some of the old tool++ setups, they were immensely pretty and functional, although I don't think any of them work with the newest version.
There are also plenty of nice and easy layouts on deviantART, some inspired by tool++'s if you want that style but also the newest foobar, and no matter which one you pick it's probably going to be at least prettier than the default skin, if it's more functional or not depends on your needs really.

Even if you don't like fiddling I would still recommend foobar as it is very good even in its default state.

I use iTunes because I have to use it for my iPhone and there is no point in using more than one music player when I have no problems with iTunes, it works just fine for me. I use VLC for videos because it works for pretty much every file format and works well besides the odd graphical glitch when I swap between windowed and full screen mode, but that issue sorts itself out within a couple of seconds and I haven't updated VLC in a long time so they've probably long fixed the problem.

Songbird, the best music player I've ever used. It is to iTunes what Firefox is to IE, takes the good features of it and builds upon them. (plus addons ftw)

iTunes on my PC, Windows Media Player on my iMac.

...No, wait, I mean the other way around!

Anyway, yeah, I just use what came with the package. It plays my music, rips my CDs and I can create my own playlists - what more do you really need? Well, iTunes can automatically import or convert any song to the AIFF format, so I guess that's a plus too.

I use iTunes, pretty much only because it's the first one I used, and I've had no problems with it.

I also have an iPod, and they sort of go hand-in-hand.

I use itunes on my iphone because i dont want to carry another music player with me.
Also it is good that the music stops when there is a phone call or message.

WMP never has given me any reason to distrust it. Although it does tend to crash if I try to go back to my music library from watching a video but since I watch anything visual only with vlc I never have that problem in day-to-day use.
