The Scream masks.

Since the guy that wore them always wore a flowing black robe, chances are, the only thing you'd see coming out of the darkeness would be a floating, bleached-bone white face of a man in perpetual torment.
Scary Movie fixed that - about 15 years later >.>
Funnily enough, that made me regress back into fearing the masks.
Well seeing a drug addled killer kind of dulled the fear for me, not to mention his guise was being mentally retarded - after that I couldn't take him seriously, then again Michael Myers still creeps me out - but the AVGN helps with that

The transforming scene in Snowwhite. When the queen drinks that shit and becomes that old hag.

Always skipped that part.

I couldn't go into the Slender Man thread. I skimmed over the OP and it creeped me out for the rest of the night. I haven't known paranoi/anxiety like that in a long time.

But as a child the thought that my parents, my cat, my floor, anything could turn into the T-1000 and slice me up. I was terrified of that.

Sirens used to scare the shit out of me as a kid.
Also, whenever a bee or wasp got too close to me I jumped up and fleed in terror... needless to say I was a lot of fun at barbeques.

One thing that still terrifies me are thunderstorms. Many years back, a lightning bolt cracked down about 50 metres away from me, and it felt like God was using me as target practice for the new apocalyptic whip he'd bought that day. The thought of getting struck by lightning and the accompanying feeling of utter helplessness still mortifies me.

Vampires. I was afraid they would pop out of dark places and bite me unexpectedly. If only Twilight had existed when I was kid, then I would find them lame instead of scary.

Same here. I hate that sound the eye makes. Actually, almost the whole game creeped me out when I was a kid. Gorons, the spiders, the zombies, Ganondorf... Even the guards looked creepy. Like they were about to rape someone. And don't get me started on Majora's Mask...

I was attacked by a dog when I was younger, so I suffered some cynophobia for some time.

A lot of scenes from the old Indiana Jones movies used to freak me the hell out.

Like the Ark of the Covenant scene or the part where that one guy drinks the wrong Holy Grail.

Gave me nightmares for a few weeks.

Just remembered something, did anyone ever watch Watership Down as a kid? Its the animated movie where all those bunnies get brutally murdered. Scarred me for life watching that.
