Let me first say a story that makes some bloody sense. MW2's plot had so many holes it could be used as a sieve. And add some length. I cleared that thing on hardened in 6 hours.
In the multiplayer, things should be removed rather than added, but here are my personal suggestions:
- fewer killstreak options (nuke should definitely be out, and big guns like the AC-130 and chopper gunner should either be automated, deal less damage, or not light enemies up like christmas trees without a friendly running UAV in the air)
- the noob tube should lose the aiming sights to balance it's splash kill capabilities
- get rid of last stand as a perk, so many player use it as their main way to get kills. keep it as a deathstreak, though.
- double the reload time for akimbo weapons to balance their capabilities a bit more
- get rid of those high cmaping points. If IW wants to put new angles to get into a base, make them have a less open firing or less accessible.
- And dear God get rid of those cheap camping points such as Wasteland's middle room
And regarding your suggestions
- Leave the number of guns as it is, but remove pointless extra guns (wa2000 is just a weaker version of the m14 EBR)
- Extra melee weapons would be redundant and pointless. The game already has 3, the regular knife (free, powerful, but slow on the rebound), the tac knife (fast, but needs unlocked pistol) and the shield (blocks enemy shots, but does less damage in melee) that's a pretty good balance
- A dedicated hacker can break through any commercial system. IW would need to have military level electronic defenses to stop hackers
- Having all headshots kill would render all sniper rifles or heavy weapons useless, as everyone would just take a weapon like the ACR or PP2000 and nearly continuously stick to it.
Remember: fun, balanced gameplay over realistic gameplay.
