Perfect Dark on the N64. I'd have been 7 or 8.
I played a ton of Starcraft64 as well, and given the day and age of when it came out I'm still surprised it wasn't rated M on the N64 at least. Always thought the sounds of people dieing on that game were a little above T. Not to mention all the blood.
I have never been restricted in watching a movie/playing a game. I've always had a "reap-what-you-sow" deal with my parents. For which I am very grateful. I was(and STILL AM, sadly enough) way less sheltered then other kids my age.
One memorable quip I enjoyed sharing with my mom was:
"Please don't become some sort of mad serial killer."
"I'll try not to."
"Thanks."

The frist game? Iono, but the first M game I ever played was either Diablo (if that was M) or Diablo 2 :3
Halo. My sister bought it and asked me to co-op with her. I'm pretty sure I accidentally screwed everything up and then got repeatedly slaughtered. Eh. Story of my life.
First one I played was some really obscure, low-budget, crappy game that involved fighting cars and live-action cutscenes. I played it for about two minutes and never saw or touched it again, but I'll always remember it as my first M game.

It was a demo of Thief II. I remember feeling very strange after blackjacking an innocent civilan and tossing their body in the fireplace.
