Splatterd by my own Banshee in Halo 2. I jumped out of it on Ascension and it crashed up above me. Then while I started sniping it slip back down and landed on me.

Twilight princess.
There was this level with a lot of lava, and I was struggling for some reason, and I finally got to the other side. And I was making Zelda jump up and down in celebration., and I shook wiimote on accident and he fell in the lava.

I was playing Mortal Kombat Mythologies: Sub-Zero (yeah, yeah, I know. Spare me.) with a few friends, each of us handing off the controller when we died. I'd managed to get pretty far into the first level... Or maybe it was the second. Whichever one had the wind boss...
Anyway, I was at the part where you're jumping from stone pillar to pillar, and killing the guys that are waiting for you. I jumped to the last one and tried to uppercut the guy, just to switch up my "jump-kick-jump-kick" strategy.
The hit connected, but for whatever reason, I took the damage, and I was sent careening off the edge of the platform to my death.

It was completely horseshit, and likely induced by a glitch, but we all had a great laugh about it. After all, it had simply looked hilarious. My friend called the death a "Mentality."

Halo 3. The spaceship In Amber Clad decended from the heavens, blowing debris everywhere. I was killed by a flying Covenant tank.

I lol'ed.

COD 4 single player, the mission were you have to escape after sniping that bloke....i plannted a claymore by a door and backed away thinking "ill get them sneaky russians" promply backed into a large swimming pool werupon i was setup-on by wild dogs....needless to say there wasn't much of me left for the chopper to pick up. There was also Hitman: blood money when i threw a mine over a wall hoping to blow som1 up, not realising that i missed and blew myself up....and about 15 civilians.

OH OH OH!! I remember one from like a million years ago, back when I used to play Duke Nukem 64 with my old friend.

Him and I were in the level that you fight the second boss in. It's a large, hollow, cylindrical room with weapons all along the walls. Back then, there were no team matches, see? So we used to just chase-down a Dukebot and if we killed each other, we didn't care. Basically, my friend fired a heat-seeking rocket to the Dukebot, but that was just as I killed him, so the rocket came towards me instead. I made about 5 laps around the room, jumping, screaming, laughing my ass off. I even jumped over top of the guy who shot the rocket, and it just careened over him. Eventually, I ran into something and got blown-up.

Unreal Tournament '03 for me. I was on dial up back then and was struggling to keep up with everyone else since I was 10 behind when the thing loaded.
So I grab the Redeemer/Mini Nuclear Missile and fire at the nearest group down in the swamp of Antalus map, and then hear someone behind me firing. I turn, and not only does the mouse spin my vision to the ground, but the dial up hadn't even rendered me firing the missile, so It fires it right at the ground.

In other words, I die like a noob thanks to dial-up who literally shot himself in the foot with a nuclear warhead.

Epic Failure of the year, as far as gaming goes.

In Halo 3 when you have to fight two Scarabs at once. I was on one of the Scarabs when one of the Banshees dogfighting above randomly came and landed on me without warning, then flew off.

In COD4 multilayer I tried to throw a grenade out a window at the same time as I got shot into Last Stand. Somehow I threw the grenade while in Last Stand but since I was lower it bounced of the windowsill onto me. Sort of like Last Stand and Martyrdom at the same time.

Well...

Back when dinosaurs roamed the Earth, and I played EVE Online I was know as 'Friendly Fire' because I killed more people that I wasn't suppose to kill in pvp than I did people I was suppose to kill. Ouch, long sentence.

Anyway, I was in charge of a gang of 30(ish) and we were sitting outside of a POS (player owned station) with this group of my alliance mates. We were preparing to warp on some hostiles to kill them. POSs are (usually) heavily defended structures that have plenty of firepower to fend of small - medium size gangs. This one was leading more toward the medium, so it made a great safe haven while we tracked them down.

We were all talking over TS (Teamspeak) while one of our buddies tracked down and set up a spot for us to warp on the hostiles. Over TS, some guy who is new to our alliance, starts talking about I-don't-know-what, but the end result is that we all get super pissed at him. Eventually, because I am in charge of this gang I tell everyone to lock him, just to scare him a little. For those of you who don't know, when you die in EVE you loose you ship and half the stuff on it, so dieing in this MMO is much more harsh than in others.

He doesn't really get the message though, so I give the order to open fire. Now... I had forgotten completely that we were sitting at a POS... and that they have a tendency to shoot anyone who shoots friendlies. I was the first that opened fire, so the entire force of this POS is focusing on me. After that, I gave the order to warp off because the POS was shooting at everyone who attacked him. About half of the people were smart enough NOT to shoot, although looking back I should have harassed them about not following orders :) Four more people died.

I lost a fairly pricey ship too. For those who might know this game, I was in a HAC, an Eagle to be specific, and this was back when HACs were new. This set me back a little bit. I tell ya though, I almost fell out of my chair laughing so hard at my own idiocy. It was awesome.

Flying around in a helicopter, On GTA 4, before finally having to jump out because it was exploding, landed on a roof top with only half health (No body armor), and being hit by the helicopter, BUT NOT DYING, then, I was pushed off the building, not very high up, and landed on the ground, STILL NOT DEAD, then finally, I got shanked by a bum, which killed me...

My friend and I were, to say the least, completely amazed that we had survived all of that, but EXTREMELY pissed that the BUM had killed us...

Halo 3.

Me and a friend riding along on a mongoose in a team slayer match, and he has the rocket launcher. Good times ahead, right? Well, someone throws a grenade at us and it passes us by, then blows up, knocking us off balance. This was just as my friend shot the rocket, so it hit the ground right beneath us, we exploded, and the whole thing flew into a tree.

EDIT: I completely forgot about GTA 4. 2 here.

One, I was racing down a street on a motorcycle, chasing a limo I was supposed to kill. For fear of losing it, I looked behind me whenever it passed by and continued to shoot. Well, one time I hit a high curb. I was sent careening into a brick wall, fell a story, and survived. That is, until a car backed over me out of nowhere, going about 50 MPH. Backwards.

Second one is right after I get out of the hospital, I run across the street and get nudged by a car. I fall onto her hood, slide down a bit, and she proceeds to slowly drive forwards, and I slide under the car bit by bit. She ran me over at about 1 MPH. Slowest death ever.

Battlefield 1942 all the way! I remember on DC Lost Village - with the Desert Combat mod, no less - I was flying the Blackhawk with my clan, practising getting from point A to point B and such, when some clanmate accidentally shoots me through the windscreen of the helicopter before getting in. I was in a joking mood, so I got in another Blackhawk and pursued the guy who was on foot running through the town while firing my machine guns at him just to make him scream (which you could hear through voice chat).

The rest of my clan were in another chopper flying towards their drop-off point near a factory, and I paid no attention to anything other than chasing the lone soldier down. Suddenly I hear the sound of another pair of helicopter blades: my own clan's!
I pulled the helicopter up just in time to see four guys sitting in the other helicopter, and promptly span out of control into them, killing eight people of my own team in the process. This was followed by much laughter and verbal abuse.

Ah... good days.

I crashed my airplane on a freaking bomber plane.

Now, understand that in this game, if you can see the enemy plane in detail, he's too close to you. We're talking very long range fights. And I managaged to crash in midair.

in mercs 2, i wasent Exactly Used to Laser designators yet, so i launched a bunker buster, and then stopped putting the Mark on it, at this point it lands randomly, in this case right on top of me

in rainbow 6 vegas me and one of my freinds were assaulting a room in a terrorist hunt, so we both decide to bounce grenades of the top of the window frame, at a angle, the grenades bounced out through each others window and blew us both to hello

in soul calibur 4, i managed to ring myself out using talims move set by jumping over my target and out the side

Assassins Creed i was on one of the High ledges, trying to jump off into a hay bale, i ended up falling a fair deal and face planting myself

My friend was playing GTA: San Andreas and he was flying a plane pretty low. He accidentally fell out of it somehow (mispressed triangle) and fell a good few feet to the ground. He had the tiniest sliver of health left.

"I'm still alive!" he yelled, little more than a second later he was hit by a car.

Oh god, we laughed for about 20 minutes straight, and giggled about it for months afterwards.

Back in the days of Final Fantasy VII, I wanted to fight the Midgar Zolom on my first pass by that area - instead of taking out the time to catch a chocobo and avoid it. After all, why would they put a ridiculously impossible-to-defeat monster there if there wasn't some way to defeat it?

So I spent quite a while running up and down the landscape, buffing up my characters. In this quest, Aerith was an absolutely necessary party member as her limit breaks at that time not only healed the party completely, but also made them temporarily invincible (At least I believe that was what occurred; it was a long time ago and my knowledge of the special attacks in a wretchedly over-indulged rpg are limited.)

Finally, while with a friend to give guidance, I fought the Midgar Zolom for the n'th time, a few times, and achieved the goal - to have Aerith survive the one 'insta-kill' move. After all, if she survived, she could revive Cloud and I could continue my battle efforts.

When she survived, my friend and I shouted, "She lived, she lived!" only to have the Zolom tail smack her immediately afterward. ... "She's dead, she's dead!"

That was my most hilarious death.

Needless to say, I would not be defeated. I spent much more time grinding and finally was able to beat the Zolom. And I did, indeed, beat the thing on the first time through. The downside is that a second later, it initiated another battle without giving me any time to run away. So I did give up with crushed spirits.
