Mine would M16, Cod spawns, Ninja Gaiden, or Kaizo Mario. All of those make me want to cry, and have. It takes a real man to cry. Where am I going with this...

Yep, I still don't have the achievement for protecting all the buildings, and I refuse to lower the difficulty.

People who ***** and moan about people with higher ranks having better weapons in multiplayer (seriously, that is like saying "No fair!" to every PvP match or gunfight there is).

Anyone that uses martyrdom. I still jump away from dead bodies in games like TF2 because of that.

I managed to get past an area in The Longest Journey without picking up a vital object.
Then much later I found out and had to start the game all over.

Playing an RPG where the first few hours involves a compulsory tutorial in which the most excitement you get is killing rats, or other vermin for 1xp each.
Starting RPG sequels with the same character who has forgotten EVERYTHING since last game (Thank you Black Isle for BG2, skipping both of these stages in one fell swoop.)
Games in which stealth is not possible, eg, when enemies have 360 sight range, or take a close range headshot from a silenced weapon, and DON'T DIE, but alert ALL OF THEIR BUDDIES, or in RPG's where the reward for sneaking up on someone for a silent melee attack is that you do a pathetically small damage bonus, and are promptly killed for not being a warrior or mage.
And people who are not spies in TF2, but stare at you and act suspiciously when you spycheck them, and make you waste time and ammo, when they could just show that they aren't a spy.

When you know exactly what your opponent is up to (in a strategy game for example) but can't figure out a way to stop it. And everything you do just makes it worse.

Grinding. GAH.

I was playing FF3 and didn't have enough magic users for the final boss, and got completely pwned. So I had to go level-grind for hours to increase the job points after I switched classes for a couple characters. It was so frickin' annoying.

And I'm playing an FPS for the first time, and slathering baddies coming up from behind and eating my head is driving me nuts. I daresay this will be the only FPS I play.

In a great number of turn-based RPGs with random encounters...encountering an enemy or group of enemies you'd much rather not fight, and then finding yourself unable to immediately flee.

-Beggar ladies in Assassin's Creed. The lepers/drunks I can deal with because you can avoid them easier, but the beggar ladies get in your way no matter what you do (and I truly feel weird running away from them but it's the easiest way to avoid them).
-Archers/snipers/ranged-attack enemies that you can't find
-enemies that explode into clouds of deadly gas or suchlike
