In terms of physical violence, the worst i've seen are Rurouni Kenshin, Elfen Lied and Ninja Scroll, Hellsing has a fair amount of violence, but it actually suits the purpose of the anime and it's not too gratuitous.

I'm thinking Elfen Lied. It was also the first anime I've seen.

There was a scene when the main character tore a 6-year-old girl in half with invisible tentacles and left her to die in her brother's arms. And the fucked up thing is - that wasn't even the most brutal scene in that anime.

The entire anime was just difficult to stomach, even for an adult. It was actually psychologically demanding to watch it. But I did anyway because the story was quite unique and interesting.

I am afraid to say I saw it because it was quite literally gore porn with lots of brutal sexual assault.

I didn't watch it willingly, it was apart of someone's birthday party oddly enough. I would describe its content but I am afraid the FBI would come busting down the door.

Otherwise I consider Akira probably the second must brutal anime I ever saw. The detail was just utterly shocking to me. Basically a similar reaction one would feel to real violence I guess from my summation.

The most violent anime...
I watched quite a few of those.

The top 5 are definitely (not sorted):

Afro Samurai - over-the-top fighting scenes, quite fun actually to watch
Hellsing Ultimate - even more over the top ;) - Ultimate senseless gore and fighting sequences with people being literally ripped to shreds.
Gantz - once again people get killed here in many various ways. Unfortunately ending is very poorly done, the anime doesn't compare at all to the first part of the manga... (at some point manga also had gone bad)
Deadman Wonderland. People dissolving in acid - enough said.
Shigurui - A anime about a samurai school. One of the most violent pieces I've ever seen (like brain coming out of cut in a half skull). A lot of physical violence, sometimes very hard to watch. I don't recommend it though, the end is way too abrupt (like a book with missing half of pages) - most likely they were hoping to end the story in second season that never appeared... well, you can always read the manga though.

Others, that can be also counted as quite violent:

Elfen Lied - it more about psychological drama than actual violence... end the scenes are done rather poorly, if animation was better it would definitely had much bigger impact
Berserk - not really violent, until the last episode - I know that a lot of people will disagree here but I consider most of the scenes as quite normal among the mature seinens
Basilisk - great fighting scenes and a sad ending. Basically two ninja clans fight against each other to decide which kid will become the next shogun at the present shogun order. I quite liked it, but the plot wasn't too ambitious.
Zetman - not really a good anime, but its fun to watch sometimes. A monster fighting other monsters for justice. Quite confusing at some parts.
Jin-Roh the Wolf Brigade - its a anime movie, but its very realistic. An Alternate history type anime about militia.
Ga-Rei Zero - about an anti-demon squad. Overall quite good, but the ending of first episode might leave you very confused.
Shiki - it had only a few violent scenes (spoiler: like the doctor slicing up his newly undead wife to find a way to kill vampires). Its more focused on drama and existential questions than on violence itself
Jormungand - an anime about a weapon merchant and her bodyguards. Its more a comedy type anime, but it still can get pretty serious ;) The ending sound cue - "She is loco" - describes it very well ;)
Texhnolyze - very dark anime but also quite strange... main character finally decided to speak at the end of the 9th episode. Still, quite good to watch but don't expect a happy ending.
Ergo Proxy - another dark themed anime. Hard to tell though what was that about to not spoil it too much.
Wolf's Rain - not too much violence there, but its a very good drama and its very sad, so I feel I can mention it in this category

Animes mentioned here that I don't consider as too violent:

DBZ - If you consider it violent, you shouldn't watch any of the animes mentioned in posts here
Claymore - I don't know why so many people mentioned it here, in this anime violence is very toned down compared to manga. The cuts are mostly quite clean, I don't remember anything too hard to swallow in it. And the ending was sooo bad... I highly recommend reading the manga instead though its still on-going.
Baccano - once you know most of those people are immortal it kinda looses its impact. There are tortures scenes though. But if in the end you don't get really hurt can you still call it really violent?
Higurashi No Naku Koro Ni - I watched only the first season, but I don't consider it violent, its just hard emotionally. Mainly about how hard is to change someone's fate.

Hokuto no Ken!

Seriously, exploding heads via pressure points, HOW HAS ONLY ONE OTHER PERSON HERE SEEN IT?!?

The movie shows more gore IMO, but the original TV series is good in it's own right. I used to quote Kenshiro saying "you're already dead" in Japanese every time. Awesome. Just watch some if you're into violent anime at all.
It also spawned several hilariously awful live-action efforts from various countries. You have been warned.

or Urotsukidoji - legend of the overfiend movies. For very different & disturbing reasons.

You kids and your Naruto. Pfft.

I'm so old....

This isn't anime, but Saya No Uta is a contender fo' sho'. Literally a world made of gore, a couple rape scenes, multiple murders, themes of genocide, insanity and a guy kills his wife and kid by stabbing them in the throat out of fear because he sees them as monsters...

Yeah, it's fucked up, but it's a damn good story. I loved it, but I feel like a psycho for my intense interest in it.

Probably Berserk. Not only is it violent, but on the rare occasion for an anime, I care for the characters enough to be shocked when I see their demise. The manga is miles better, of course.

This post basically encapsulates everything that mystifies me about Elfen Lied. I've often wondered if, maybe if I'd gone into Elfen Lied expecting to hate it, I might have liked it (hell, I might have found it interesting enough to finish). Between the fact that I didn't really give a crap about anyone in it and the yes-we-make-hentai-and-nothing-else level animation, I was totally disconnected from everything. I was just bored.

When I'm emotionally invested - and when something is crafted with talent - then I'll be as freaked out as a show wants me to be. When Hei breaks the fingers of...Christ I can't remember that girl's name...in Darker than Black, that had way more impact to me than anything in Elfen Lied.

I also don't understand how...er...maybe I should stop talking about Elfen Lied.

For ultra-violence it would have to be Karas.
It was made for the 40th anniversary of Tatsunoko. It's only 6 episodes long and still makes no sense to me, but, damn it's great.

Well, there is always Elfen Lied. That will always be on my mind for "brutal". I normally frequent comedies, so I don't usually watch much stuff aside from that. Its not that I can't take the horror (I actually kind of find it quite invigorating) its that I prefer the happiness and laughter of a comedy over the solemn depression that comes with horror.

Now if only there was a comedy-horror anime. Now THAT would be something that I'd immediately pick up and run with.

- Hellsing Ultimate (Fanfucking tastic Anime)
- Baccano (Few select brutal scenes but a well done anime)
- Berserk (Need I say more!? It's so damn good)

- Elfen Lied (Brutal, but that anime's story can go fuck itself.)

And last but not least.

Honestly?
Grave of the Fireflies

While It has relatively little bloody violence, watching two kids slowly starve to death is pretty brutal.

Not sure if it was ever made into an anime, but the manga of Battle Royale was possibly the most fucked up thing I have ever read(at least in the top 5). Hunger Games was a freaking pinic in the park by comparison.

Gantz and Berserk are the two goriest animes I've seen personally. I haven't watched Elfen Lied, but it has a reputation.

Most of the ones I would have pointed out have already been mentioned,
in particular Ninja Scroll.
But one that seems to have passed under everyone's radar is Narutaru.
Let me tell you, do *NOT* watch that unless you have some serious tolerance for
seriously *fucked* *up* violence....you will never look at test tubes the same
way again. Seriously. What's worse is that the first episode of the series looks
all cute and pokemon-like, when the brutal violence starts it's a jarring change.
Oh, and some of the most fucked-up acts of brutality are performed on grade-schoolers...
by each other. Yeah.

There was another one that I only saw a tiny bit of (on purpose, the tiny bit
I saw made me want to both avoid and forget it). I can't remember the name, but
it was about a guy in an armored suit who wandered around a post-nuclear wasteland
fighting mutants--the mutants all had really disgusting sexual deformities and the
violence level was above even Ninja Scroll.

45 posts and nobody's felt the need to mention Hokuto no Ken?

Seriously?

I feel old.
Heh heh, I remember the days when all anime fans had seen it, because there was so little else available!
Remember when having a couple of tapes of Ranma 1/2 and the first Project A-Ko movie meant you had the
very best anime collection for miles & miles around, and the staff at the video store would ask you questions
about it? Heh, the good/bad old days....

Anyway, while Hokuto No Ken taken on it's own would seem to be very violent I'm guessing that it hasn't been
mentioned because it just pales in comparison to most of the other things brought up in this thread.
