Dissapearing bodies. Why can there only be three dead bodies on screen at a time, yet a game can have over twenty living enemies shooting at me? Do ragdolls physics really require more processing power than living enemies with movement animations and AI? Games that particulrly piss me off with this are Mass Effect 1 and 2 due to it being pathetically obvious. Bodies dissapeared as soon as you turn around or there are more than three. I still love those games, but jeez that annoys me.
This bugs me too. I mostly noticed it during the 'shocking' terrorism level in MW2, which would have had way more impact if you weren't killing the same 3 people over and over. And also if all the bodies stayed. It ruins any amount of credibility when you look around to see all the carnage, and the airport is completely clean and tidy, with nothing to indicate that it had just been full of bodies.

Same as in any medium- Underuse of character development. If I'm basically piloting a robot, or (If I'm literally piloting a robot) the characters I'm interacting with are just cardboard standies, I CANNOT enjoy the game.
Oh, and of course escort missions. It's gotten to the point that if a game gives me a compulsory escort/defence mission, I just turn it off and never play it again. Which is a shame, because I thought HAWX was quite fun for the first minute or so it could resist giving you one.

Escort missions.

I have an eternal burning hatred for them, especially when it turns out you never needed the character in the first place or they die anyway

This too.

Oh, and disproportionate females. Seriously, Give a toothpick watermelons for jugs and she will fall over before she can even take a step.

People these days who cry about Betas being full of bugs....good god the opening first few hours of Reach Beta and the slam that got from "fans." That's the reason why people need to get some dirt rubbed in their eye.

Or SC2 Beta with people complaining about stat wipes...IT'S A FREAKING BETA!!!! SCORES DON'T MATTER AND NEITHER DOES YOUR PUNY LITTLE DOG-TITTED LIFE!!!

Now that's out of the way...the dingy effect games have these days, sure it's good with some games, like fallout 3, but if the main color wheel is brown, then why not just make the game retro and do it black and white? At least that's a good idea.

Being forced to keep weak NPC's alive through hostile situations. Usually this comes in the form of escorts, but it happens at random times as well. My companions in fallout 3, not to mention the damn caravans, getting shot to pieces within a matter of seconds was always obnoxious.
Yeah, for awhile I kept reloading saves to save my partners, but happened so often that I just let any partner die.

Lack of imagination. I know, unspecific, but it neatly sums up all my worries.

Lord, fucking this. On a strong second place finish: Useless fetch quests. You know, the kind were you're acknowledged as the savior of the world, you need something from some nobody, and that nobody insists that you put your quest to save the world on halt to get him a loaf of bread. What the hell, man.

Having to pay for DLC that's already on the disc!

Any company that pulls this bullshit can go fuck themselves.

NPC's who all look and sound the same...I'm looking at you Fable (1&2) and Oblivion, and I'm sure there are others, those are just the ones that immediately come to mind. It lessens the immersion quite a bit when you're surrounded on all sides by adoring clones who have the exact same voice.

For me, it's introducing an interesting element, then making it near impossible to use for the next 3/4s of the game.
You mean like Breath Of Fire Dragon Quarter?
Pointless battle conditions annoy me (I don't mind conditions, just not the pointless ones done by the developer in an attempt to be different)

2 things:

1)framerate drops like in NBA2k10

2) escorting characters who cant defend themselves or don't have survival skills(eg. little girls)

Characters with no interesting or emotional back-story, I would care far more about them if I knew they had a partner or a couple of kids waiting for them to return...

Artificially increasing difficulty. One of the prime examples of this most recently was in the Bonfire of the Vanities DLC for Assassin's Creed 2. The game basically does this: "Remember all those neat tricks we taught you to pull of stealth assassinations? Yeah? Well screw all that. You can't do any of them here. Oh, and you can't be spotted, but let's fill up your notoriety so the guards are hyper alert."

It was infuriating and not fun at all. The first time in the entire game I actually chose to use the pistol, as it was the only effective way to deal with most of the assassinations. Find a group of random idiots walking even remotely where you need to be, swing the camera wildly until the horrible lock-on targeting hits the target, and then floor him with a pistol shot.

Again, not fun.

A thousand times this, I hate guarding NPCs with no sense of self preservation.
This, and dodgy scenery. By this I mean anything you can get your in game avatar stuck on without being anywhere near it and making yourself an incredibly large, bullet magnetised sitting duck. Lara Croft spends half the time getting her massive tits caught on bits of wall every time I play a Tomb Raider game. Although this could possibly either be because I'm genuinely crap at TR games, or because every game I've ever owned in the series has glitched out to the point it's impossible to carry on.
