A piece of fan fiction can be taken seriously, such as with Wide Sargasso Sea. It's essentially a piece of fan fiction of Jane Eyre, and it's pretty much treated as canon. Which is annoying, as it's quite bad.

[spoiler/] There's a kind of porn that originated in Japan, called henta (I think that's how you spell it). Its normally characterized by young girls, with unproportional boobs to their body and age, being pretty much raped by tenticle monster kind of things. Henta is not limited to tenticle monsters and is often know as anime porn. So a connection between fanfiction and henta is not farfetched, and in darker parks of the internet (ie Newgrounds)its not uncommon to see some of your favorite cartoon characters having sex. I'm tempted to say as soon as the first letter gets typed, but I am aware there can be good ones out there.
So, I'll say when an established character gets involved in a non canon romantic relationship. As soon as you bring romance into a fanfic the entire thing goes to hell.
And sex is when you've gone past the line of no return and are now jumping off a cliff.

The best fanfics, or at least tolerable ones would be ones that exist in a universe but do not involve any of the main characters or even any important supporting ones. The only fanfics I could stomach are ones that use the logistics of the chosen universe, but have original side stories that have little to no bearing on the actual story. Anything else risks (and often times involves) major out of character moments.

I read a fanfic of Borderlands, which tried to portray the reasons the protagonists were hard up for cash and hunting for the Vault. And it showed the Siren as so desperate she turned to prostitution. That's not the kind of character she is goddammit!

But anyways, that's the perfect example of shit you DON'T do. Hard speculation in a demeaning fashion can seriously piss some people off. I mean, Borderlands didn't even have that great a story (and the worst ending, IMO), but c'mon.

There's no such thing as too far, I say... but writers who simply pair up characters tend to be boring as all hell. Cloud Mows the Lawn, though? Hilarious. Cupcakes? Hilarious.

When it's published.

Example: Baldur's Gate novels. I've only started the first one but it's the game only novelised with less NPCs and numerous subplots. And suddenly it's canon. What?! It wasn't even written by a single author. Also, any Sherlock Holmes book not written by Doyle. Yes, some are not that bad but I've read fanfiction that was actually good. It was still fanfiction.

Otherwise, not many things. I consider it a good exercise in writing. You take a character you only see in specific situations and try to adapt it and yet keep it in character in others. It's a good exercise even if you only read it since it's possible to learn from other people's mistakes.

The problem with your question is that it makes no sense; "going too far" doesn't mean ANYTHING, especially in the context of a work of fiction. It could mean too far in terms of ratings (such as highly violent material in a work aimed at young children) or in a legal sense (actually attempting to publish and receive money from fanfiction is the only way I could conceive of it going "too far"), but otherwise the wording is perplexing. Fanfiction can certainly be bad, both in general writing skills and any buthering of the source material (Sturgeon's Law applies to fanfiction same as anything else, though it appears moreso because there isn't any screening process), and on rare occasions can cause spontaneous bleeding from the eyes, but there isn't much cause to worry about it "going too far".

Slightly relatedly, I laugh both at the myriad of people here who dismiss all fanfiction entirely, as well as those who go straight to the typical "warnings" of things like My Immortal, Cloud Mows the Lawn and Full Life Consequences as if they A) aren't obvious troll fics, and B) are in any way representative of fanfiction as a whole. I can't remember all of the fanfiction I've read that was not only a wonderful representation of the setting but quality pieces of literature in their own right (which I won't list because I will probably be here all day), which goes back to your original concerns about writing fanfiction.

It's just like everything: it will only be a serious work if, first of all, it is treated like a serious work. If you only view it a fanboy's fantasy, then that's all it will be. If, however, you view it as a work of which you can be proud that only happens to be derived from a fanboy's fantasy, that's what it can be.

Pretty much sums up the reasons why I don't like (most) fanfiction. To expound on this a teeny bit, make sure the dialogue is in-character. Their "voice" should be consistent with how they have been portrayed.

Of course if, like you said, you're using only your own characters that's a whole other thing. I guess in that case, make sure they talk similarly to how the other people in the established world/universe have been shown to talk. So in Halo for instance, you never hear people use tons of slang or swear excessively, so either of those would ruin the 'Halo' aspect of it a bit.
That falls under #4.

there was this one minecraft fanfic called 100 days to mine yea thats going to far WAY TO FAR I will post the bit which has disturbed me
She was obviously struggling, but to no avail. She had been cut off in her last scream by a gag that John had put on her. John was on the bed, kneeling above Julia. She was struggling, but it wasn't helping. John was unbuttoning her shirt, going down the line. I knew I had to time this right, but I didn't want to risk Julia's safety... Or virginity.
keep in Mind the charaters in said fanfic are 14 years old AND ITs MINECRAFT
I would also like to add they are making an in game movie of it
