Uncharted 2. I think 7 playthroughs or so. It's so easy to get into and the gameplay is quite frankly, perfect. I see no way to improve it.

Mirror's Edge.

Partially because its very short, largely because its a fucking incredible game and I love EVERYTHING about it in every way (except the damn length), and will play it over and over unitl ragnarok.

I love replaying Okami. No matter how many times I play it, I seem to find new things even though I've seen it all before. The atmosphere throughout the whole game is amazing, and I always love every second of playing that game.
I like to revisit games I played as a kid, but they don't feel as exciting anymore, so it's mostly the newish games I go back to.
Oh, also--Pokémon Blue Mystery Dungeon for DS--I keep going back to that one, never FULLY completed it (as in completing all the after-the-storyline things) but I do go back to it alot.

a lot of the games listed above are ones that i played through quite a few times....one that wasnt mentioned was "Sewer Shark" on Sega CD.....i dont know why i did it, but it became almost like a warmup exercise before gaming, i could run through it in about 30-40 mins(which i did hundreds of times)

I've done Ocarina of Time runs since I was around 9 or so, 100% runs, speed-runs, 3 heart runs, you name it! Probably around 65~ total playthroughs in the last 10 years? Second is probably Demon's Souls (currently on New Game + 12)

Dragon Age: Origins... all the origins, a couple duplicates because I was having trouble getting queened. No clue how many hours all that ended up with - it was months. DA2 gets the second place, but that's only because I figured out that I could see some of the more minor variant cut scenes by going to youtube instead of playing it through again. ;)

I had a sticky note with relationship saves noted on it stuck to the computer so I knew where I had to go back to without replaying the whole thing. Saved a lot of playthroughs...

Observation: That is alot of play-throughs.

Sadly, I have not played it as much as you. I've done it 10-14 times. I still play till this day as well. It's one of my favorite Bioware games along side Jade Empire.

yeah my life is sad..that game got my socialess life through that summer like it was no ones business.

jade empire is also a gem, i still remember the trailer for it on KOTOR for the original xbox, i was screaming at my dad to drive me to best buy to let me buy it.

When I play KotOR I sometimes just watch that trailer for Jade Empire. Then I play Jade Empire all the way to completion again. That game is amazing. I would really love a reboot.

Civ IV. Easily. Not sure if RTS/TBS and FPS should count for this. If they do, Dawn of War and Star Wars Battlefront should be on there too.

Quite a few contenders for second place though. Baldur's Gate 1 & 2 each got a few playthroughs. Morrowind and Oblivion got a couple. GTA III, Vice City and San Andreas each got several playthroughs. (GTA IV, not so much) Saints Row 2 has gotten several, both solo and co-op. Katamari had several.

reboot? shit i would settle for a sequel

regardless, with those gameplay mechanics and the overall story arc, they could make one HELL of a god damn game in modern times with what they had going..

Diablo 2.. I'm not a replay person generally speaking, but the item hunting caught me in its evil little grasp.

There are several I've wanted to replay, mainly RPGs which promote how wonderful their choice systems are and how many different paths there are, but generally speaking all that comes down to is slightly different conversations or reasons for the same missions.
