I think my choice for creepiest voice in a video game has to go to Prophet of Truth in Halo 2 because he's just so... calm. He orders genocide of entire races without even raising his voice and in general he made my skin crawl with his evil actions and his eerily soothing voice. I remember all of my friends wanted to be the one to kill him.
It's a shame he didn't have the same voice in Halo 3 because it kind of lessened his appeal as a villain.

I would say Mike Patton just due to the range of creepy that he can cover, it is rather jarring to know that a man with a voice so absolutely terrifying can actually be human.
