The actual worst idea I've seen... Gosh, whatever Disney is doing with TV shows lately. Case and point from back then would be Gargoyles.

Mostly everything. Phineas and Ferb is actually quite clever.

For me it has to be lawn darts. They were basically oversized darts with oversized sharp ends that were thrown in the air, and that always came down point-first.

Before the pilot ejects, explosive charges blow off all six twin rotor blades so he can soar through their airspace without being decapitated. Theoretically, at least.

First, I've actually seen Atomic Annie, so unless it was a very large and well made prop that is indeed what it looks like.

Second, it's hard not to count a nuclear weapon as a fail when it can't even launch a shell far enough to keep the users safe from fallout.
