Disgaea 2, had 500+ hours in it at that particular save. I had just spent 12 hours working up some stats and battling through Item World at 4am.

I was saving and for some reason, my treasonous left hand hit the power button before the save was done. Aghast, I turned the console back on and confirmed that, yep, I had ruined my 500 hours of gameplay with a bit stupidity that comes from being very tired.

It was months before I spoke to my left hand again, the Judas.

This, and Burnout/Full Auto. Crash, then wait about 10 seconds to respawn. And by that time you're in dead last, about 5 seconds behind first.

This one isn't about me being unlucky but the guy trying to sneak up to me in CoD4 (PC). I was attempting to kill an enemy in a 2 floor window which had a slanted roof/canopy type thing hanging out from above it. I threw a grenade up too high, it then bounces off of the canopy type thing, flies back behind me and kills a guy who was trying to sneak up and knife me. I survived. Sucks to be him.

Just about 5 minutes ago I got into a situation in final fantasy tactics advance A2 where it became mathematically impossible to win or lose, because the final boss's AI glitched but his regen was too damn high to kill him...

Fire Emblem. You can calculate the odds and prepare for the worst but if you have an unlucky day you're screwed no matter what. Take me for example, not the worst case but I missed TWO 95% hit chances in a row just to get my own guy killed the next turn against an enemy by a 3% Crit chance hit.

TF2, Spy.

Pyros seem to have a beacon on me, and run straight towards me (even though I have cloak and dagger and stand still) and flames. Rockets, randomly fly and hit me in the face. Snipers headshot me out of nowhere.

It's hard out there for a spah...

Being spawn-camped once in CoD4. All my guys were in the same spot hiding from the campers, and since you spawn next to your guys sometimes, I spawned 5 consecutive times and died within 1 second of spawning each time. Thankfully, on spawn #6 I popped up on another part of the map and got them back oh SO good!

I think my worst luck, was purely of odds, when I rolled for a rare item in WoW, with a /roll 9999.

I got a 4.

Don't remember. But I can think of some silly times in just about any game with lava and enemies who interrupt jumps. "I can make it!!! OMG!!!!!!! NOOO!!!"

Devil May Cry 4: Bloody Palace mode. I've never beaten Bloody Palace with Dante yet, and I was really damn close. I was at floor 101, Dante vs. Dante Shadow. I had half a health bar left, he had a pixel. I stuck him with a few Lucifer darts, then he went into Royal Guard and started chasing me. Normally he can't ever catch you if you don't stop running, but the dart exploded and he used a Royal Release and killed me! I still haven't beaten Bloody Palace with Dante yet.....

I can't count how many times I've played an online game, been given warning of imminent incomming artillery, thought to myself "I'm perfectly safe" and then being killed when a chunk of concrete from the sewer I'm in falls and hits me in the head (or however the game wants to explain it).

Personally, I've had problems of late in Dawn of War 2. I'll have a brilliant strategy ready and execute half of it (i.e. use Banshees to cover my guardians while a shiruken cannon team sets up to suppress the oncomming stream of tyranids). I'll have step 1 and 2 down, only to accidentally select my shiruken cannon team when telling my banshess to move back to the line. Inevitably this results in the tyranid swarm quickly rushing forth unopposed (except by the guardians, who though quite deadly, are constructed of paper and squishy bits when it comes to actually getting hurt). Basically, this means I might, at best get a portion of my army out, usually having to sacrifice a squad or two for the greater good.

I also occasionally have command stick where I select a unit for maneuver and orientation and then it fails to register the full command. Since much of the game actually revolves around very short windows of opportunity, this often results in disaster. What's more, I can never replicate it intentionally so I don't know what I do to cause the problem.

I've got a few, but this one sticks out:
Left 4 Dead is a big boiling pot of bad luck, even for the AI, which is the subject of my story:
No Mercy, end of the Finale, by the staircase, me and my sister were playing, me being Bill, her Zoey, with Louis and Francis being controlled by AI.
Now both L and F had been particularly stupid that game, and as a result, Francis was dead, and Louis was incapacitated by a Tank, twice.
The helicopter had just arrived, so I picked up Louis (by the radio/window, incapacitated) while my Sister had gone off to the helicopter, and when I finished picking him up, I started off as well, expecting him to be in tow, however, halfway up the ramp, I had been informed by that damn text that he had been pounced by a hunter, by the time I react to this, I am already on the pad, but I turn around anyway, and start back, halfway down, I notice he is but 2 FEET FROM WHERE I PICKED HIM UP AT, and I hear the final rush coming, accompanied by more text saying he was incapacitated again, and without a Pipe bomb or Molotov, and with half my health, you know what I did?
I turned around and said "Fuck it, Louis can die."
Narrowly making it on the chopper before being eaten, the credits start to roll, and the text starts off, going "In memory of: Francis, Louis"
To which I replied "THAT'S WHAT YA GET FOR BEIN' STUPID, YA DUMB BASTARDS!" in the best wheezy old man impression I could pull.
After that we had a good laugh.

While playing GTA4 I was in huge fire fight with about 2 dozen drug dealers. I was talking cover behind my car in the middle of the road. after I had killed all but three a random car comes form the left trying to get away from the gun fight and runs me down my mistake

L4D has many of these moments, the AI directer seems to be bend on creating trauma.

Quick saved my way through the original half-life only to find that I'd accidentally quick saved at a spot where I'd been cooking a grenade for a bit too long with about 25 health. There was no way to recover and I had to start completely over.

Motorstorm games spring to mind!
Midnight Club for me. I remember doing a race for pinks, I'm in a muscle, he's in a tuner. I barely pull ahead when we reach highway part of the race, and I floor it. I use some nitro and go soaring down the road. Shortly afterwards he drops off my radar. I keep going, but after a few more seconds he's getting back onto my radar, and making ground. I had my car maxed for top speed so I could get ahead at parts like this! Needless to say, made a wrong turn near the end, and he beats me because he caught up on the highway. I was not happy.

In PC FPS games, I have a terrible habit of quicksaving right as some invisible bastard of an enemy AI lobs a grenade at me, so I get stuck in a perpetual cycle of explody death. <_<

Also, in Fallout 3: Operation Anchorage, I'm trying to take down Jingwei, who is a ***** with his health and damage resistance. I don't even know how it happened, but, at some point, I must have accidentally struck one of my comrades, because suddenly all the American soldiers turn on me for no reason. Maybe Jingwei ducked behind one of them, or I caught someone in a frag grenade explosion, but, either way, I'm taking on a boss & both the Chinese & American armies all at once. I managed to kill a few, but I died before I managed to get Jingwei, what with having no stimpacks in there. <_<
