Neverwinter Nights 2. Have cleared an entire room of skeletons, sacrificing both of my companions to get that far. Just me and one skelly left. Should be easy right?

Need for Speed Most Wanted (and I'm not even referring to the hilarious rubberbanding here)
I hit my opponent in a 1 on 1 race so he crashed and just kinda stopped moving for about half a turn. Then he (I'm not kidding) for some reason fell from the fricking SKY after 10 minutes, blocking my sorry arse and speeding away to eternity and beyond. Ok, maybe you could consider that rubberband AI but this goes too far.

Or doing a bunnyhop on GTA:SA and dying from hitting the traffic lights above me.

Probably getting utterly stuck in a cave in Pokémon Blue and accidentally saving. 20 hours of work and a great line-up lost...

Also, trying to get the acheivement Annex:Now with excecution rules (win a match of annex on all the Flashback and Combustible) on GoW2; it's amazing how retarded some people can be.

I'm playing Team Fortress 2, it's 2-2, I've got the enemy intel and is just outside the intel room, but when I entered the room I ran right into a enemy heavy-medic pair.

Timesplitters 2. Respawn right in the middle of a hailstorm of minigun bullets and the occasional brick thrown by a duck. You cant last long!

One that happens all too often is that I'll have a pack of TNT, throw, miss, only for the AI to stick theirs right on my face...suffice to say, at least we both go down.

playing halo 3, i was with 6 of my friends in social slayer match on the Pit, and the 8th player the game found was lagging vary badly. After being sniped by the 8th player i respond in side of him and neither of us could move or fire. he left the game after about 5 min of just sitting there and i still could not move, my friends though this was so funny they left me therefor the rest of the match

I was playing Fallout 3 when I enoutered some Radiers with a Fatman.... After using all my ammo(just finished a big mission) I had both my legs crippled and head with no stimpacks... Then 2 Rio Gurau(mutant bears) decided I looked tastey so we played cat and mouse all through out the wasteland(very hard).

Mine is a moment of the worst best luck ever.
I was playing fire emblem shadow dragon on hard mode, my cavalier was severely wounded and a pirate moved into attack, if it hit it would kill him. He missed but then the cavalier scored a critical hit, killing the pirate but opening him up to another attack from another pirate, which hit and killed him... Under any other circumstances scoring a critical with a 1% chance would be great...

I was playing Fallout 3 on my second playthrough ( I wanted to be evil this time) and I started walking to Arefu for the 'Blood Ties' mission. About halfway there, I saw an explosion nearby, and realized that I had walked into the sights of a Super Mutant Behemoth with a missile launcher, with a regular Super Mutant about 30 feet behind. Keep in mind I'm only level 4 at this point, with my strongest weapon being a Hunting RIfle with 15 bullets. After finally killing the two Mutants, with about only 50 HP left, I keep walking and I see a Raider fighting a dog. I decided to bludgeon the dog to death with my baseball bat first, then chased after the Raider, since he ran after two hits. Imagine my surprise when I turn a corner and see 4 raiders waiting near a collapsed bridge. I died.

Having no less than 5 grenades land at my feet in STALKER Clear Sky, in the only piece of cover not occupied by something that wanted me dead.

Getting 20 hours into the game, shooting a Bloodsucker in the head, and then for no reason other than poor poor programming, my game freezes, and shuts down. Did it everytime, even if i didn't kill the bloodsucker. Have they patched that game yet? I want to frigging finish it, and i'm not restarting (again) if i know I'm not going to be able to finish it.
