Being left in space, in a suit with life support, but no possible way of rescue. Or buried alive. Basically any death where you're alone, with no hope of being saved, and you have to simply wait it out, slowly going mad.
Or burning to death. The pain must be unbelievable.

Drowning

I would say drowning, I can't remember where I heard it but apparently drowning is pretty much a painless way to die so I wouldn't think of it as a worst way.

But anyway, I'd say getting shot/cut somewhere and bleeding to death.

Edit: Just remembered that scene from The Green Mile, when the guy doesn't wet the sponge.
I think that's added to my worst possible way to die list. If I had one.

I've heard that's actually a very peaceful towards the end. After you stop flailing I mean and your lungs are nearly filled. I imagine cold water would also dull your nerves to the point that you wouldn't notice the cold.

As for my entry...being crushed to death. Not just being run over which would SUCK but if you fell into a sinkhole or, quicksand and were covered with debris, it would be slow and somewhat less violent but still. It would be horrifying.

worst painful - anemic dysentery

worst best - ordering the death of three thousand innocent people and then fighting a guerrilla war for nearly 10 years living in a almost prison away from your home in a high security complex before hearing the sounds of helicopters realizing your hiding place has been uncovered and then cowering before a highly trained special forces team bust in shoots you in the head loots the complex and leaves and then dumps your body almost unceremoniously into the ocean.

Anything slow, drawn-out, and painful. Humiliating is also a plus, but not a requirement. Unusual (though not necessarily humorous) would also help, but it needs to be so bizarre that very few would know what to do in such a situation. Unfortunately, I just ate, so I kinda had to stop reading your wiki-list after a bit.

Drowning is something I've always feared, although anyone who has watched the tortue scene from Law Abiding Citezen will understand why that comes a close second... I mean... he bragged about cutting off the dudes peni-actually... I don't want to remember or think about that happening to me.

seriously though, the worst would be, nailed to the floor and slowly having your intestines pulled out of your body to eventually bleed out after a few hours.

Well, if you die of thirst you get to feel all the cells in your body shrivel up and die. I've heard it's pretty much the worst way to go as far as suffering is concerned.

One that I thought would be especially bad though, came when I was watching Death Note. Person's on live tv, and suddenly suffers from uncontrollable flatulence that builds in intensity until they literally crap their guts out in front of everyone.
