For me, its the sound the witch makes when she's about to wake up. Freaks you right out.
I get really tired of that taste the blood song in DMC3, and the "welcome to hell" when you fight the joker character in the special edition. Also, whoever selected the voice actors in DoW:SS deserves to be shot. "I feear no evaaal forr i am feear innncar-nate" ARGH! The originals were so much better.
And because I thought "worst" thing was going to be scariest:
I always got freaked when you hear the thump thump of the bloodsuckers in STALKER - especially in the dark where the bastards are invisible.
Alma's giggle, the pitter patter of face sucking fingers in the AVP series, and worst of all:
Silence.
You hear silence, and you just know you're about to get fucked up (tm) severely by some big bastard.
I also used to trip out about "Gutentag" in Wolfenstein 3d

The only bad sound I've ever heard in a game before is when you're playing Red Orchestra and you hear the sound of MG bullets pinging of the scenery near you as you dive for cover, but it's too late and you're dead.

and I guess it's the only Intro Conversation I remember from all of the people from the game.
