I've played a lot of games with QTEs. The entire God of War Series, Indigo Prophecy, etc. Usually if you fail you instantly die, but there's some time to react. I've recently acquired Red Faction for the PS2, and my God it is hard without autoaim. When the mercenaries start appearing with Railguns, they can shoot you through walls, and insta one shot you. For example, right when I thought the room was clear and was at 100 HP and 100 Armor, I got instantly railed by a guy in the next room. What about you? Worst surprise insta death?

Fallout 3, caususly strolling around the wasteland, I had my music turned down, it was late at night, when my screen goes red and half of my health is down, i turn around to find a deathclaw standing over me, and another attack and I die.

I was pretty pissed off.

OT: Once I was playing WoW and we stole the Horde blimp to Northrend. As soon as the area finished loading I fell through the ship and the map, falling for a good 3 mins before dieing. The fall wasn't instant, but the death was. The circumstances were also confusing.

I was playing once and I got stuck in a wall. I could still turn and shoot, but I couldn't move. Cool thing was, that I was also invulnerable. I finally died when I tried to jump and I rocketed out of the wall into the skybox and fell to my death.

Not exactly a quick time event, but upon my first playthrough of Half Life 2, I managed to duck under one of the combine crusher-walls just before the Entaglement chapter. It slams down, I stand there hoping for a cool secret, then suddenly: Boop, dead. Beep, beep, beeeeeep. Made me sad. ):

Oh man this Super Mario rom hack is hard as shit. I'm almost done though, I see the exit! YES 5 hours well spent! Hmm its just a single jump to the exit, this looks easy. No need to save, I got this in the bag!

All right, I'll actually contribute. I would say probably the bears of RDR. They have killed me yet but they did kill my horse which irritated me because I was in the middle of nowhere and had that same horse for quite a while...RIP my loyal stallion, you will be missed. But then, there seemed to be a glitch because, after I skinned it, I looked up and he was standing right in front of me alive once again!!! It was kinda weird that he was standing over his own dead body...IT'S A MIRACLE!!! That's a good Argo (yes, he's named after the horse from Shadow of the Colossus). Here, have an apple.

Also, this. Didn't Rockstar learn that people wanted to be able to swim? I mean, the first GTA games you couldn't swim, they got complaints, and then they fixed it for the future games. Why didn't they apply that logic to RDR? Oh well, they gave me "Dead Eye"...kinda an acceptable trade if you ask me.
