I gotta go with Hiroshima. It was a sunny day and everything.
Hiroshima, Hiroshima, Hiroshima...

Seriously, what about Nagasaki?

They were hit with an Atom bomb as well!

Everyone talks about Hiroshima, but Nagasaki gets nothing.
Well, yeah. But it couldn't have been as much of a surprise for them. They at least knew the bomb existed.

Thresher Maws in Mass Effect can go fuck themselves.
Agreed. Getting one down to almost dead just to have it pop up under you and instakills were the worst.
Drive in "really" big circles and don't stop for anything if you want to live....... plus actually shooting the damn thing and all. On the bright side in Mass Effect 2 you get to give the ultimate humiliation to a thresher maw by killing it with small arms weapons.

I'm pretty sure the two bombings (Was "Fatman" that dropped on Nagasaki I believe) occured pretty close to one another. What it was exactly probably wasn't even known to them...
...
Maybe they thought it was Godzilla?

I'd do that and it would still pop underneath.

Worst instant death(s) ever?

S1: Tomb of Horrors.

Now this is not a video game but it still counts. The party walks into the entrance chamber, all we can see is two statues with a chest between them. The rogue and arcane trickster search the chest finding the false floor, pry it open and grasp the handle. A 30 foot gap appears all around them and they fall into a pit landing on three poison spikes apiece causing their instant death.

Thirty seconds later, the monk and fighter both are walking along the tunnel when the floor drops out from under them; they both fail their saving throws and die instantly.

Only I (Druid) and the chaotic evil sorcerer/red dragon disciple are left. My friend tries to walk through a wall using an ethereal nature spell and she is attacked by 2 class IV Balors and is killed naturally.

I continue walking alone down a corridor when i tip a counter weight and the tunnel i was walking down slopes suddenly and my gnomish druid fails all her checks and tumbles down into a pool of lava.

5 minutes into the dungeon we are all dead.

Red dead redemption.

Cougars.

That is all.
not as bad as the ninja bears

I was recently playing metro 2033. In one of the more heavy stealth segments I was up on a bridge/railway/pipes section. Which of course hovered over a giant green puddle of whoops you back up to far death.

Bad Girl in No More Heros. I thought I had the advantage. After having my brains splattered across every wall I learned different.
Hehe, just played that fight again the other day. Very subtle difference between actually having a large advantage over her, and "I'll one shout you if you come anywhere near me"

The worst instant death in history is Megaman (in any incarnation) dying when falling on spikes. I'm a big Megaman fan, but to this day I still do not understand why a man covered in metal will die if he falls on metal spikes. I also do not understand why I scream and act like a maniac whenever this happens. I'm a very calm and collected gamer, but get sent into a rage while playing through Megaman games.

It isn't exactly death but...

I remember playing MGS3 and, at The End part, I was there, standing on the middle of the field with the Directional Mic, trying as hard as I could to hear where the hell The End was.

So I hear his heavy breathing and I'm "OK, now where is he?" I look at the sound's direction, trying to see where he's hidden, but I can't really see anything, but I know he's getting closer because, damn, the sound is getting louder. And there I am, wondering where he is and suddenly BANG! Tranquilizer dart on the back of my head!

I fall down and he takes me back to the prison, miles away from there.

That was surprising and incredibly lame on my part...
