There have been so many times that I have been fighting a boss or been in an intense firefight and had to reload. This breaks up gameplay at in the best situation or ends up with you having to start the level over again at the worst.
One example was the time when I was playing the old Ghost Recon, was storming what can best be described as a desert fortress with a bunch of terrorists in it. I thought I had a full clip and charged in learning that I only had one bullet and immediately had to reload, consequently having every terrorist in the fort shoot me multiple times. Another was while playing Republic Commando. I was holding the bridge of the ship, charged at a Transdosian merc and etc. you get my drift.
