Worst way you have died in a videogame

On my first time playing Rainbow 6 Vegas, dropped from the Helicopter and well..It went a bit like this
"Right, now what button does what *presses buttons* ok, that ones the grenade...wait a minute, oh shi-"
I had managed to kill myself in the first 5 seconds of the game.

The worst way i've ever died is thanks to COD4. I was playing Sabotage on Countdown, i spawned in on the opponents side and didn't seem to be noticed, then died two seconds later for no specific reason. in the killcam the opponent threw an intended magic nade which turned 180 degrees directly towards me...im not making this up.
Getting my ass beat buy jiggly puff when I was new to ssb. Wah I should have at least beat jiggly puff
I didn't really die because it was a bail in Skate, but it was awesome.
I was skating along, trying to do the best bails I could, when I hit a truck, which launched me into a taxi, and back to the truck. I fall on the road, and just when you think it's over, the same taxi comes barreling down the road and clips my head and I crumple into almost a ball shape.
I broke 12 bones that day...
(The most I've broken is around 14 or 15 on the megaramp).

Gears of War chainsaw death is pretty brutal, but the 1st game that came to my mind was Eternal Champions on the Sega Genesis CD. (The CD version was a MAJOR upgrade from the cartridge.) That had deaths even more brutal than Mortal Kombat. I'd say the worst is a finishing move in the jungle level. My guy was punched, went flying back, and landed in a giant cauldron in the background filled with a bubbling red liquid. My guy was screaming & tried to climb out, but his scalded flesh fell off his bones, and his skeleton fell back into the pot. Very shocking the 1st time that happens.

the free Windows re-release of Death Rally. My car was pretty damaged, but I had got almost a full lap before the other cars, and on the last lap, when the finish line was visible on the screen I accidentally push the Windows logo button on the keyboard, which minimizes the game and opens the start menu, as fast as I can I switch back to the game, but it think I've been holding down the right arrow key all the time and does not register that I don't, so the car just spins around in a circle in front of the finish line, a car catches up and shoots a few rounds on mine and I die. That was pretty annoying.
I have died in bad ways in a lot of other games too, primarily FPS' and RPG games, and of course Dead Space.

In destroy all humans I got my wanted level all the way up, hid in a pool until the heat died down, then hopped out, noticed all the enemy cars were still there, blew them up, then got hit in the head by one of them and died because I clipped through the ground and drowned.

On a halo 3 custom map, my friend was blocking the teleporter on top of a building, so I shot a rocket at him, he moved so I went through the teleporter and appeared at the top of the building with my own rocket heading towards me.
