I've said this to a lot of people, but Metal Gear Solid 2's ending blew my mind.

Now, most people use the term "It blew my mind!" to describe something amazing. However, MGS2's ending LITERALLY did blow my mind. Meaning that there was an actual minor implosion that took place in my brain after watching it. I even turned my PS2 off and on again constantly to make sure I was getting all of this stuff right. I couldn't think straight for days.

The Milkman Conspiracy level, in its entirety, from Psychonauts.

"To promote niceness. To make the world prettier. To share candy with everyone. To obfuscate the true nature of the Milkman. To protect the Milkman at all costs. To destroy all who would harm the Milkman, or threaten to reveal his secret objective."

The ending to Assassin's Creed was a WTF - it wasn't particularly well done (I'm talking about the point where we get back to Desmond, Altair's ending was fine) and literally nothing was explained.

AC2's ending however was well done and it does make sense.

I learned to never punch the piano player in the bar in Armadillo (the game is Red Dead Redemption btw). When I hit said 'piano man', Marston freaked out and was instantly teleported to Cholla Springs.....in the fucking sky. :/

Either skinning an animal while on a horse and getting shot into the air of red dead redemption or surviving a .50 cal shot on call of duty

I was playing Spirit Tracks a few days ago, and, while in the spirit tower on level 10, I told zelda to put the big key in the lock. She goes to do that and at the same time she was going to throw it in, she got hit by the floormaster, resulting in the key getting stuck to her head. She can't pick it back up or get it off by my weapons or the hands so I had to turn off and redo the section. Still not sure how it got stuck there.

In the first level of Alan Wake, you have to make your way to a gas station and call for help. The bad news? The Gas Station is on a street between two tunnels and a cliff. Both tunnels are blocked off by Crashed Trucks and are impassable. This was most likely used to keep the Player in the area.

The weird part? As soon as you hit the phone to call for help, a cutscene occurs with the Sheriff coming by herself to the gas station in her police Cruiser.

How the heck did she get past the trucks?
