Falcon's Talons in Ninja Gaiden Sigma 2. If I go by the way I kill the last enemy, he cuts my arm off, stabs me through the throat with one claw, and decapitates me with a backward roundhouse kick. Not fun.

that would be a quality claymore +3 in demon's soul 
by seeing what it does in that game and assuming the character has a high strength stat, it'll be pretty quick lol

That would be the Kraken Bolter and/or the Chainsword (would be the Thunder Hammer if the game hadn't switched out my loadout for the boss quicktimeeventbattle thing).

I get killed with my silenced, laser-sighted pistol from Deus Ex. Probably a headshot, so at least it was painless. And of course I wouldn't see it coming.
I was playing Shadowman and using the Marteau, a jaw bone that sends a few fiery sparks flying about, may be a painful death
The silenced SMG from Halo 3: ODST? As long as it's in the hands of a trained killer, and I get a clean(ish) death, I'm okay with that. There are worse ways to go. Like a Gravity Hammer from the same series.
