I'm going to fail to stay up till 3am tonight to play Fallout NV at launch because I've been up since 5am so I could study and not fail the midterm I had today.

I didn't apply for any scholarships even though I had a 4.0 GPA the year before, so that's a pretty big fail.
Another fair is I got lost for 3 hours on my way back from Walmart when I went to buy Rock Band 1 band bundle with a friend when it first came out. To be fair, I didn't know my way around the city I was in, I got cut-off before I could get to the exit I DID know and didn't know how to get back to it, and I got so far off off course that I ended up in another town.
In games, I failed when I respawned in Just Cause 2 and the MG I had spawned in the main character's groin area, pointing up. That was pretty funny though, especially when I shot the gun.

While staying in another country, I was riding down a hill on one of those big luggage carts. The kind that you see at airports. Anyway, I slipped off, fell onto the ground. I turned over, the cart hit my feet, and fell on top of me. The side pole on the cart slammed right into my nose, breaking it.
You know why it's such a huge fail? Because, in hindsight, there was a very easy way I could have avoided breaking my nose.
