Everyone gets their own personal universe.
They are fully aware that they have full control over it.
They can change anything at will and even interact with other people in their universes.

Everyone in the world now has a health bar above their head. It gauges physical health only, and displays it above their head. On death the health bar crumbles into dust, it can not be thrown away and it will always find it's way back to it's owner like having a balloon tied to your back.

I would become god, that is a change to a single aspect of the universe, is it not? I would then go about giving human beings an actual purpose, as we are all just dust with no reason to exist other than the very fact that we do.

Also, I would cause random chaos and punish atheism with confusing random things to prove my existence, mainly because having my level of trolling capacity raised to that of a deity, would be fun as fuck.

That's two things! Make your choice: indestructible time-and-space-traveling adventure boxes, or lots of silly hats. Just be aware that Valve and Loading Ready Run have already faced the same question.

OT: Make the universe game-like in substance. What I mean is make it big and populated by interesting people, places and things, no matter where you go; and make it so that everyone gets the chance for a Hero's Journey to explore these places, come to peace with themselves and help others do the same.

I'd change the way evolution went down on planet Earth, so that instead of apes turning into the dominant race we'd have a race of giant sapient earthworms ruling the planet. Earthworm Jim as well as the worms of Worms have all proven that worms can be incredibly efficient and resourceful.

Also, earthworms are hermaphrodites, so I just solved all your gender issues. You're welcome.

Karma and morality would be literal, so bad people would have bad things coming to them inevitably and good people wouldn't have diseases come to them etc.
