rainbow six 3. me and a few mates were addicted to terrorist hunt. we had it on realistic with friendly fire on, just because it can provide some extra laughs. anyways im lining up a headshot from distance with the sniper rifle and just as i pull the trigger my mates head pops into my cross hairs. he didnt believe me that it was an accident but it was funny as hell

Halo 3 Team Slayer on Valhalla. I toss a plasma grenade at a Banshee that's heading to the other base while running, It misses it by an inch, and then it sticks on me. Cue stream of obscenities.
My dumbest ones likely involve Kyle Katarn and the tops and bottoms of elevators. 'D-hey, maybe if I jump up into the roof of this one it'll go to a secret area!' That worked exactly once. The rest had predictable results that left one wishing for a Force power that worked against Elevators. Or that you could at least carve a hole in their ceiling/floor with the Lightsaber.
Recently had a laughable death in replaying one of the old-school Castlevanias- I was so worried about the dreaded Flea Man that when stumbling across an unexpected one I was so desperate to avoid being humped to death I leapt away... right into a pit. Less embarrassing in that I'm sure everyone who played those has done this at least once.

Gods so many, especially in online games. The most recent one I can recall is in Zelda Twilight Princess. I just entered the Fire Temple, and died before even encountering enemies. I failed the easiest jumps, not once, but about 4 times in a row, for no apperant reason. I facepalmed, and promptly cleared the dungeon without any trouble. Gods that Goron guard mini-boss was a crapload harder than the actual boss...
In Prince of Persia games where it seems that certain areas have Jupiter-esque gravity, where a fall of something like ten feet will make the Prince lay flat in the air and bellyflop onto the ground, killing him instantly. It always makes me laugh, but a part of me is sure I've survived many a similar fall.
once, in left 4 dead i was the last guy alive in the group on hard diff. and a tank was running loose. i hid in a closet to try and escape it. do i need to say how it ended?
Was playing in an Arma 1 PvP tounry and i was reversing out of the way form a ATGM BDRM firing like mad. I avoided all the missiles and then "ploop" i reversed into a lake sniking my tank and killing me...
Cod 4 MW i was trying to not get killed by a guy as i was the last guy left saw a well and went "hey i can hop in hear and use it as cover" so i jumped over the wall to find a 50 foot drop at the other end.

The cops were closing in and we were both laughing/cheering at how he was going to escape. Then.... it happened. He was a few feet from the boat, almost there, when a police car FELL ON HIM. Roof first. The single most stupid and hilarious death ever.
I know it wasn't really a result of stupidity, but it was still hilarious and I felt that I had to share it.

He'd gotten into a high wanted level police pursuit in GTA4 and in an attempt to escape he down to the docs and spawned a boat. The cops were closing in and we were both laughing/cheering at how he was going to escape. Then.... it happened. He was a few feet from the boat, almost there, when a police car FELL ON HIM. Super Mario Bros. 3. I forgot for a brief second that to float you had to keep hitting jump instead of holding it down... right as I approached a pit.
Died one night in Orgrimmar during a city raid (Pre-BC days) and decided id use my mage instead. Now not only was my warrior being corpse camped by the entire horde but by the time i got there with my pvp mage the battle had ended. Tried rezzing the warr but 2 shamans and a rogue thought it would be fun to live in the GY. Lame.
i was travelling in a group on cod:wow we were all snipers. my death was on the second round
FIRST ROUND
the first rounds we destroyed the enemy. I was leading the pack... were were on daytime makan the sea side village. were were sneaking in the water all of us with gily suits on. WE were in the middle of the water under the building.
the enemy were all hiding in the building at the far end of the map .... they were in sight.
one of my man was to far up out of the water . he was a good 5 meters from me. he shot. the enemys had snipers to. the moved i say where everyone of them were one was hiding in the bushes next to the building. one was hiding in between canoes on the shore.
2 were standing in the building. one was standing out side of the building on the corner. i what my springfield ready to shoot when they spotted us. they shot my friend who was 2.5 meters from my right. then the shot my back up down the water 10 meters from me. i was probly next. but i was hidden really well. i shot the man between the canoes then the man in the bushes then the 2 in the building then the last one by the corner.
I fought that round by my self.
