There's two that always elicit a chuckle, no matter how many times I see them:

The first is from one of his reviews (not sure which) where he mentions time travel, and shows the Tardis flying across the top of the screen, and the sound effect coming out from behind it is just 'miniminiminiminiminimini.' It has to be the best textual representation of a sound effect I've ever seen.

The second is from his review of Mercenaries 2, when he's talking about the Irish chopper pilot. He makes some mention of things going to shite, and shows the helicopter flying away, and the pilot is shouting 'Jesus begorrah!'

There's also one of his early reviews where he shows a woman cocking her leg and shooting a fetus out. Don't really remember the context, but I do remember that he inserted a disclaimer that said something like 'this is the worst joke I've ever done.'

Oh, yeah, and "if someone said to me 'sci-fi re-imagining of another culture's mythology, I'd immediately think 'TOO HUMAN!' and punch you in the bollocks for reminding me of it." That's not visual, really, but it still always makes me laugh.

"FUCKING HELL, DID ANYONE SEE THAT?! I'M SQUIRTING MACHISMO OUT OF MY NIPPLES OVER HERE! I AM A MONSTER TRUCK THAT WALKS LIKE A MAN!"

Every time the caption "Stop that" shows up when someone does something silly I burst out laughing. Also the "Takes guts to wear that" from Sleeping Dogs.

Hell if I know which video it is, but one that's stuck with me for a long time is of The Woman Bent Over (you know, the same woman who appears in all of these visual jokes) with a caption reading, "oh no my bum and the seeing thereof etc." I'm not sure the "oh no my bum" joke has reappeared since then, but if not, that was a killer send-off for it.

Don't remember the episode or the specifics, but the one where a guy is running forward like an idiot while the caption says "CHAERG".

I tend to type "CHAERG" when I do something particularly stupid/awesome because I like it so much.

I like the fish jokes from his first Modern Warfare review.

From the same review: Screwed the pooch so hard the pooch had to lock itself in the bathroom with a jar of soothing cream. With a cut in picture of a dog with wipe open eyes heading at high speed towards the bathroom.

My absolute favorite has to be this one from his Halo Wars review:

"That may sound like a small complaint, but small things lead to big problems, like a tiny piece of broken glass lodged in the urinary tract."

As he says this, it shows an imp standing over a toilet on a stepladder, and as soon as he says "lodged in the urinary tract" its eyes suddenly go huge. I was laughing for almost 5 minutes the first time I saw it.

The one I can think of at the moment is mentioning Bayonetta has giraffe legs thus showing her lying down with her long legs hanging out.

Nothing has been able to really top this yet as far a visual gags go.

it had a nice image to it, so:
"you want numbers? how about 4? like 4CK YOU."

- something along those lines in a review I still need to find again

The depiction of the hole in the sky over New York in Resistance 3.
A hole with a giant kitten poking it's head through whilst someone tries to shove a cork in it.

Captcha: Describe Burger King with any words
How about "Not present in my country, dipshits"?

The one that always stick with me is one from his review of Left 4 Dead. That part when he says "you are one of the last four sane people on Earth"... and one of the characters is dressed up as a clown.

And then there is in the Mortal Kombat review, where he mentions that parents are afraid twelve-year-olds will go around punch each others heads of, and that is what happens on the screen.

I just can't get them out of my head...

Captcha: Describe Verizon with any words
"This company does not exist in Sweden".

Two recurring ones come to mind that continuously amuse me no matter how many times I see them.

The first is whenever he visually communicates a character's stupidity by making their eyes spaced more widely apart than normal. I don't know what it is about the simple act of moving one eye slightly farther away that gets the point across so effectively, but it does and it never fails to make me happy. Bonus points whenever it's coupled with a dazed look at the sky or scenery, a happy or excited posture, or being placed in a combat situation ("CHAERG," which others have mentioned, is coincidentally another particularly good one).

The second is far less common, but has still popped up more than once: Sometimes, when discussing the possibility that a game's protagonist hallucinates or is otherwise living in a fantasy world, they're given the command to "KILL THE WHORES" for a split-second before we cut to the next image. Again, I'm honestly not sure why this gag tends to stand out to me over all the others, but I'm glad it does.

I like whenever he's describing the character having a moral dilemma or having to make moral choices, and the guy is sitting there holding a can of Coke in one hand and a can of Pepsi in the other hand, and looking back and forth between them.

The "there's no middle finger big enough", it also fits with everything.
Which video is this from?

I liked in DA2 review when he was talking bout the single female body model and he says something like "...so your mum has the same banging DDs as the strippers FREUD FREUD FREUD"

also "I can't speak for everyone, at least not until "The Device" is complete...

and the whole day labourers with cricket bats from the Neverdead video.

The bit from the real Duke Nukem Forever review regarding the imps trying to build a house opposite from a boat on a river.
