Also, @warpenguin... Cuntnugget has been round for a while. I swear in the past few years me and a few people I know just add nugget to something and it makes the insult way better.
I also like "Fuck knuckle", a favourite of my mum and grandma.
30 Rock had some great ones in the finale:

"Goodbye forever, you factory reject dildos"
"You soupline at a gay homeless shelter!"
"You Eastern European knock-off Mr. Potato heads!"

I really like to do variations on this little gem from Ranma 1/2 inserting 'more intelligent', 'less incompetent' or whatever else is appropriate for the situation.
