I have a few from Shadow Hearts. The first one was where I was killed by roaches 
The second one was one of the side missions at the end of the game. Sorry that I don't know how to do the whole spoiler thing so don't read beyond this point if you don't wanna know.

COD4, watching the nuclear blast growing... And waking up in the post-nuclear storm, all like "fuck, how come I'm still alive?", managing to crouch for 10 meters maximum, before inexorably falling in the dust, dead.
That was a real "WTF" sequence, just blew me away. The closest thing to blockbuster movies a video game has ever come to.

I have 2, 1: I was playing MLB 09: The Show and I swung at the third strike and the catcher missed it, so I was running to 1st and I noticed the catcher wasn't moving, I scored on the play.
2: In Halo 2 the checkpoint hit right as I was getting out of the Warthog, and I got run over by the warthog reversing and got caught in a loop of death.

First: The ending to no more heroes... does'nt get more random than that.
Second: Attacking Sheogorath in oblivion and ending up 600 feet up in the air.
Third: Ragdolling in midair in assassins creed, falling from the highest point in the game and dieing.

The end of F.E.A.R. 2: Project Origin. Even though it was such a mediocre game [sub](as you can find out in my review in the User Reviews section)[/sub] the ending was quite...powerful, especially for those that don't normally play horror games.

In episode 2, when I filled a couch that alyx and her father were going to sit down on with about 20 objects... they fell through most of them and jammed a few down under them. Then Eli's hand gets jammed on a book under him and the book flies out of the couch like a cannon, hitting me in the face and killing me.
When Liberty Prime went rogue and accidentally killed the Enclave and the B.O.S. when I first played through the last FO3 level. He did it again, but this time it was purely accidental, meaning he didn't use his lasers on them.

My friend Chris was playing Resident evil 5, collecting the shattered bits of plate to get through the door, ran through the river like a mad man, grabbed the plate piece and legged it again, just about to get to the bridge where sheva was standing and yelled "home free" and got nommed by a croc less than a meter away from the bridge, i nearly wet myself laughing.
