I'm just going to go ahead and list whole series where I have had limited to no exposure as listing individual games would be much to massive of a task. With that in mind:

Conker's Bad Fur Day
The entirety of the LucasArts adventure game canon
Mega Man / Mega Man X
Final Fantasy
Chrono Trigger
Resident Evil, Silent Hill, Eternal Darkness and any other major horror title.
Civilization
Metal Gear Solid (minus 2)
Thief
Splinter Cell and the majority of Tom Clancy games
Deus Ex
Jak and Daxter series
Elder Scrolls, Fallout and most open world RPGs in general
Diablo
Tomb Raider
Call of Duty, Battlefield, and the "modern military shooter" genre as a whole
A large majority of major PSX and Genesis titles and the bulk of "cult" SNES releases to be honest.

...and I think that all I'm going to list at the moment, although I'm sure I could extend this list into something much more massive and slightly embarrassing.

Dark Soul. I deeply regret I did not give this game another try, and that's all. I could have played Witcher 2 on Xbox 360 but it's not shame more a small regret.
You could still give it another try. When I first played it I got so frustrated a few hours in that I stopped playing. Then when Dark Souls II came out and everyone was talking about it I felt like I was missing out. So I picked the first game back up, persevered, and now it is one of my favorite games of all time. Even got 1000/1000 achievements over 3 and a 1/2 playthrus with over 126 hours played.

Dragon Age: Inquisition - Someone got this for me expressly because they thought I'd love it and I did, right up until the moment my save was locked in a completely unwinnable random battle that sees half or more of my party being instant killed by a fireball within the first few seconds of the battle. Have yet to bring myself to play through from the beginning all over again.
I pray you mean Dragon Age Origins, otherwise you have a game that's not out yet and I'm moving in with you!

OT: Far Cry 3, bought it for the hubby, he loved it and so I gave it a go, it's still sat there waiting for me to return

I'm gonna say Deus Ex HR. Enjoyed playing it, Then put it down for some reason and now I'm too intimidated by it. Final Fantasy 12 for pretty much the same reason.

Final Fantasy 6 has been on my shame list for a while. Now that I finally got a cartridge, I now need to get an old TV to play it on. I'm not emulating it.

I couldn't play GTA4 Ballad of Big Gay Tony. Rather, I couldn't play Lost and Damned because I kept getting really nauseous. I blame the color pallet. I had that slight problem with original 4 while i was playing it, combined with the fact I did not like 4. I'm too afraid to touch Gay Tony now.

Goldeneye 007. Never had a chance to play the original. When good word came out for the Wii remake, I played it once, then never picked it up again. Probably the Wii controls. I've really not liked them after Okami.

Never Finished Rock Band 2. All of my music games just sit on the shelf.

Tony Hawk Pro Skater for N64. I just couldn't figure out how to play it as a child. I still cant today.

Final Fantasy 4/Chrono Trigger for PS1. Had them on the shelf when I was a child. Tried them, couldn't understand their complexities, then gave them back to their owner.

Just Cause 2. I don't know why. I should play that.

The biggest one for me is Final Fantasy 9. I can tell you all about the story, the main characters, the bosses, the music in great detail, as I've seen videos, watched others play it. Yet I've not actually touched it myself. Here's hoping Square will re-release it on Steam sometime soon!

Guild Wars 2. I played it for a while. I tried my best to enjoy it, I really did. But I just couldn't. I found the worlds a bit unfriendly, no one was willing to talk to me, do anything with me. I found the constant updates a pain in the buttocks, as my internet is not spectacular, and so those updates take forever for me. Also each time I tried to log in, it wouldn't let me, as apparently I log in from a different place each time so I have to go and give myself permission to log in. Every damn time I tried to play this happened. I'd like to play it again, I liked what I did see of it, but bah.

Metro 2033. I've played a lot of the game, I've just never finished it. I've read the book and absolutely adored it. I adore the game, I really do. I don't know why I've not finished it, I just haven't.

Seeing how I have miss out 99% of the popular games, I got quite alot like-

LA Noire
Red Dead Redemption
Skyrim
Mass Effect 1-3 (ok I got the game but I never in the mood to played it)
Bioshock 1 and 3
Infamous 1-2 (in saying so this is at the bottom of my list)
Bayonetta (if only my bro bring over that game already...)
Fallout 3
Dead Space 1-3
Portal 1-2
Anarchy Reign (ok I heard it's average but I still want to played it)
Vanquish
Killzone 1-3
Resistance 3 (ok I don't owned a ps3)
Dark Soul 2
Left 4 Dead 1-2
Dishonour
Assassin Creed series
Resident Evil 4 onward
The Last of Us
Chrono Trigger (I played and got far into it but I couldn't be bother to finished it).
Shadow of the Colossus
Journey (well ok I don't owned a PS3)
3D Dot Hero
Braid
Torchlight
Dragon Crown
Bastion
FTL

I'm pretty sure I'm still missing out a few more popular games but if I list anymore, I would no longer consider myself to be a gamer.

Unfortunately I don't have a system to play the game anymore.

My most glaring wall of shame ornament would be GTA San Andreas. I didn't even make it past whatever the first town was called. I just got bored with it. In my mind I feel like I can go back any time and finish it, but I just don't feel like it. To me all GTA games play more or less the same way and for that matter I'd rather move on to IV or V.

Never finished:
Any GTA game
LoZ: Ocarina of Time
Super Mario Galaxy
Half Life 2
Fallout New Vegas
Any COD game
Uncharted 1
Assassin's Creed IV

All because they were super boring.

Never played:
Any Metroid game
Any Team Ico game
Any Final Fantasy game
Any God of War game

I'm sure there's more but that's all I got for now.

Hmm... Well, I feel no shame whatsoever as none of these games really grabbed and held my attention like a digital kidnapper waiting for the hostage's family's credit card information to check out, but there are a few I played and didn't finish/almost played/probably never will get around to that I've heard good things about.

Shadow of the Colossus. Got to the third boss when I determined that I just wasn't having any fun.
Borderlands. Decided after the very first bandit camp that I'd rather be playing Fallout 3 or something.
The entire Metroid series.
Any given Half-Life.
The Sly Cooper series.
Okami. Took a few dungeons before I just stopped caring.
Any Silent Hill. Not my cup of tea, but I've enjoyed a few Let's Plays of the series.
GTA San Andreas. There was this airplane mission that apparently demanded 100% perfect flying for several seconds and I eventually lost my patience, drove around shooting things, crashed into a fiery wreck, and never went back.
Anything on the Wii, for lack of interest in the console as a whole.

My big one is never finishing Half Life 2. I've also never finished any Final Fantasy after Final Fantasy 6.

To date, the ones I can remember I've not finished are

Recettear (Rather hard to beat, but I've still not given up on it!)
Half Life 2 (too boring)
Kingdom Hearts 2 (lost it)
Deus Ex Human Revolution (too boring)
Fallout New Vegas (Save file corrupted at 34 hours, couldn't be arsed starting again)
Halo 4 (too boring)
Dragon Age Origins (VERY boring)
The Witcher 2 (shit combat mechanics put me off)
Watchdogs (too boring/slowly making its way to the bottom on the river Mersey right now).

The list of games I haven't played would be too large to type out. Reasons either stem from owning the wrong system at the time, or lack of funds at the time.

I never finished Witcher 2, even after buying it again on Steam this year cheap for a second chance. I dunno, I SHOULD like this game but I just can't get into it. Its theming is far too heavy handed and lacks almost any subtlety. It's not my thing, like how I never played Last of Us because I disliked its entire universe. Also a perfectly good game with good game mechanics (ladder simulator not included) but eh... I refuse to believe the world would get that fucked, or at least continue to stay 100% fucked 20 years after the start of a zombie apocalypse. Joel and Ellie being good characters aren't really my concern.

Back to Witcher 2. Yes, you're being dark and "mature" well done, you can stop swearing now dear, you're embarrassing yourself. Oh but if you're going to swear, actually bloody swear. You're not helping yourself with this "ploughing" nonsense.

I suppose I don't like dark stuff that's darkly written just for the sake of being dark. I can just imagine a lead writer on Witcher 2 standing over someone going "DARKER! WE NEED THIS DARKER! ADD A TASTELESS JOKE ABOUT WOMEN HERE!" I can't stay interested in something that is determined to be mired in complete misery for 20 hours.
