i can't say there is any game that i consistently play. usually when i complete a game, i move onto another one. rarely replaying games and more frequently i don't finish games as often as i used to. (just don't like a lot of modern games anymore) But i do replay some of my all time favorites, mostly RPG's like:
Final Fantasy X, Legend of Dragoon, Lost Odyssey, etc

If i'm feeling nostalgic i'll pop in some older games like Mega Man 2/3, Ninja Gaiden and Castlevania.

Left 4 Dead 2. Love the fact that you can jump in, join a game, play for a while and leave whenever you can. Or you can stay a bit longer and play all the way though.

See, I don't have a lot of time to play anymore so this game is perfect for me right now.

Any of the last 3 Elder Scrolls games, interchangeably. I even cross-over my characters!!!

Sam & Max games (seasons, whatever) on the Wii are nice to sit back and have a beer with. Low stress, light puzzle, comedy - maximum relaxation.

Also, I play Majora's Mask every year or two, so maybe that counts.

Every generation seems to bring a different Go-to game for me. So, starting from the second generation (where I started) and moving on:

2nd: Centepede for the 2600
3rd: Mario 3 or Ninja Gaiden for the NES
4th: Final Fantasy 2/4 on SNES
5th: Tekken 3 on PS1
6th: Tekken Tag Tournament on PS2
7th: Dragon Age Origins

PC - I did most of my PC gaming in 2004-2006 and excluding WoW, my biggest go to was probably Jedi Academy or Max Payne

Rome: Total War. Still endless fun, after 7 years, and playable on basically any modern computer.

For me:
Alien Hominid on ps2 (fun solo or co-op!)
Hitman:Blood Money : as someone else mentioned earlier, a small playspace and your wits. replay value level : 10/10
Super Metroid: I just love this game more than words can describe
Resident Evil 2/4 : I just love em both.
Super Mario All Stars+ World: all the classic Mario platformers on one beautiful cartridge

and from this gen, find myself playing a lot of RDR and ACII

I would have to say TF2. I play it when I don't really want anything too in depth, or only have a few minutes to play.

I also am currently playing Pokemon Emerald emulated on my phone. I had never played Pokemon before, but I am enjoying it.

Probably the Punisher. I lost count of how many times I've completed it. I haven't played it in a while but whenever I just want to play and don't have anything I want to play, this is it.

I have found myself playing through Ratchet and Clank's 2 and 3 whenever my computer dies or when I run out of new games to play.

Almost certainly Europa Universalis III, I've got 740 hours invested in that game as of this moment. I have a number of games I play regularly if I'm bored (the other big one being Crusader Kings II) but EUIII is definitely the main one. Honestly I'd say I have a go-to genre rather than a go to game, I find myself playing one of Paradox's grand strategy titles whenever I can't find anything else.

Depending on the mood I'm in, it would either be Journey or Dragon Age 2/Mass Effect 3. Find all of those games pretty relaxing in their own ways.

Halo. Specifically whichever iteration happens to be the newest title in the series, though I'll admit that I've put in Reach again just for the multiplayer on occasion. Otherwise I'll usually revisit an entry in the Castlevania series.

Right now it seems to be left 4 dead 2. It's like my gaming macdonalds: familiar and always available, but usually surprisingly unsatisfying.

I'm stuck somewhere in the vortex between Modded to Hell Minecraft, LoL (if I'm in a LoL mood, which can be infrequent, but lasts a long time), Token MMO of the Day (TOR, WoW, The Secret World, Guild Wars 2, freaking Ragnarok Online...), Token Elder Scrolls Game of the Day (so, Skyrim, until TES 6 happens, or Elder Scrolls Online is so amazeballs that this category merges with the MMO one. Or I get the urge to play Morrowind again), Pokemon, and Action RPG Thing (modded Diablo II, Diablo III, or Torchlight II). I hope to add Monster Hunter to the list at some point, but I have no idea what I'm doing in that game...

For multiplayer, every day I have the same dilemma: Team Fortress 2 or Tribes: Ascend. On the one hand TF2 has a magnificent player base and variety at every step. On the other hand, Gotta go fast!

I don't really have singleplayer go-to at the moment. My hard drive is a bit constrained, so just keeping games around after I finish them isn't really feasible. Been thinking 'bout reinstalling Civ 5 though, but tho Gods and Kings DLC is so expensive...
