Ju-On: The Grudge [http://www.wired.com/video/juon-the-grudge/46435411001]
Just from the clip I got scared shitless.

Also, while I'm still on the subject of games, the scene from Indego Prophacy where you had to navigate the female detective though the asylum with all the doors open and the violently insane people are walking all around scared the crap out of me. It's done from a first person perspective and you can only see a few feet, while every once and a while a patient slowly walks across your path making no sound. Utterly terrifying.

The Dunwich Building in Fallout 3 scares the shit out of me... and anything similar in any game
Supposedly weird freaky poltergeist shit is supposed to go on in that building, but I've never encountered any. Not even a door moving by itself... *sigh*
I saw a few doors close/open by itself, and the creaking noises.

For a long time I was planning to make a UT2K4 Mod based in my High School. The game centered around a student being locked in overnight, unable to leave, he starts to notice strange occurrences described above by a few people, such as the lack of an audio track, mundane surroundings "corrupted" by an un-natural presence, ect..

When I graduated High School the project wound up hitting the shelf for a few years. Recently I acquired the UDK, (Unreal Development Kit). The engine has a special place in my heart and I had been looking for a project to play with. I think I just found it.

Tall Man?
I'm assuming he's referring to Tall Man from Trilby's Notes (it's one of Yahtzee's games).
Here he is:

He's more threatening than the b/w picture leads you to believe.
Play the chzo series. The plot is extremely tight and everything is wrapped nicely by the end of the fourth game. Some parts are suprisingly effective in mood despite being like...8 bit

Your own mind. Nothing fucks with a person more than something that hits really close to home, addressing your insecurities.

That's why, if I had to design a horror level, it would be set in a normal house. You've chased this horrible entity here, and you know it's somewhere in this dark, creepy house, because there's blood on the walls, and you swear you can see doors moving like someone just ran through them, when you suddenly come across a room that's completely dark except for the light from a game console, and a flickering TV screen that says, "Please reconnect the controller," and you can clearly see bloody handprints scraped across the floor.

Point Lookout on Fallout 3. that was a bit jumpy, not so much scary.
Dead Space freaked me too much. No other game has really scared me like that did. at least, not that I remember...
